经掌侧入路锁定加压钢板固定术治疗老年桡骨远端骨折患者的效果分析

Analysis of the effect of locking compression plate fixation through volar approach in the treatment of elderly patients with distal radius fractures

目的探究经掌侧入路锁定加压钢板固定术治疗老年桡骨远端骨折患者的效果。方法选取2020年1—12月本院收治的78例老年桡骨远端骨折患者为研究对象,采取随机数字表法分为对照组与观察组,每组39例。对照组行手法复位联合石膏固定治疗,观察组行经掌侧入路锁定加压钢板固定术治疗,比较两组围手术期指标、腕关节功能恢复优良率、术后并发症发生率及生命质量评分。结果治疗后,观察组掌倾角、尺偏角分别为(7.15±3.16)、(20.16±5.44)°,均大于对照组[(−2.77±4.26)、(9.89±2.31)°],桡骨高度为(10.46±2.13)mm,高于对照组的(8.77±2.84)mm,骨折愈合时间为(9.51±1.05)周,短于对照组的(10.32±1.62)周,差异均有统计学意义(P<0.05)。观察组腕关节功能恢复优良率为94.87%,明显高于对照组的79.49%,差异有统计学意义(P<0.05)。观察组术后并发症发生率为5.12%,明显低于对照组的20.51%,差异有统计学意义(P<0.05)。观察组社会功能、躯体功能、角色功能、认知功能评分分别为(80.55±4.89)、(82.32±3.84)、(82.02±5.13)、(81.45±4.86)分,均高于对照组[(75.15±4.82)、(76.20±3.36)、(75.55±3.84)、(77.32±3.56)分],差异均有统计学意义(P<0.05)。结论经掌侧入路锁定加压钢板固定术治疗老年桡骨远端骨折效果确切,可促进患者腕关节功能恢复,并减少术后并发症,增强康复效果,提高患者生命质量,值得临床推广应用。

Objective To investigate the effect of locking compression plate fixation through volar approach in the treatment of elderly patients with distal radius fractures.Methods 78 elderly patients with distal radius fractures admitted to our hospital from January to December 2020 were selected as the research subjects,and they were divided into the control group and the observation group by random number table method,with 39 cases in each group.The control group was treated with manual reduction combined with plaster immobilization,and the observation group was treated with locking compression plate fixation through volar approach.The perioperative indicators,the excellent and good rate of wrist joint func-tion recovery,the incidence of postoperative complications and the quality of life score were compared between the two groups.Results After treat-ment,the palmar inclination angle and ulnar declination angle of the observation group were(7.15±3.16)and(20.16±5.44)°,respectively,which were higher than([-2.77±4.26],[9.89±2.31]°)of the control group,and the radial height was(10.46±2.13)mm,which was higher than(8.77±2.84)mm of the control group,and the fracture healing time was(9.51±1.05)weeks,which was shorter than(10.32±1.62)weeks of the control group,and the dif-ferences were statistically significant(P<0.05).The excellent and good rate of wrist joint function recovery in the observation group was 94.87%,which was significantly higher than 79.49%in the control group,and the difference was statistically significant(P<0.05).The incidence of postop-erative complications in the observation group was 5.12%,which was significantly lower than 20.51%in the control group,and the difference was statistically significant(P<0.05).The scores of social function,physical function,role function,and cognitive function in the observation group were(80.55±4.89),(82.32±3.84),(82.02±5.13),and(81.45±4.86)scores,which were higher than([75.15±4.82],[76.20±3.36],[75.55±3.84],[77.32±3.56]scores)in the control group,the differences was statistically significant(P<0.05).Conclusion The effect of locking compression plate fixation through volar approach in the treatment of elderly patients with distal radius fractures is accurate,which can promote the recovery of wrist joint function,reduce postoperative complications,enhance the rehabilitation effect,and improve patients'quality of life,which is worthy of clinical practice.
