止血带联合宫缩剂在高危产妇剖宫产术中的应用效果

目的探究止血带联合宫缩剂在高危产妇剖宫产术中的应用效果。方法选取2018年1月至2021年5月于本院分娩的80例高危妊娠产妇作为研究对象,随机分为研究组与对照组,各40例。对照组采用缩宫剂卡前列素氨丁三醇止血,研究组在对照组基础上联合止血带止血。比较两组术中出血情况、血流动力学指标、术后血红蛋白下降值及泌乳质量。结果研究组术中出血量小于对照组,术后血红蛋白下降值低于对照组,差异有统计学意义(P<0.05)。两组心率、收缩压、舒张压、呼吸频率比较差异无统计学意义。研究组缺乳、乳汁不足占比均低于对照组,乳汁足量占比高于对照组,差异有统计学意义(P<0.05),两组乳汁基本够量占比比较差异无统计学意义。结论高危产妇剖宫产术中采用止血带联合宫缩剂进行产后止血,能显著降低产妇产后出血量,且不影响产妇的血流力学指标、泌乳质量,值得临床推广应用。

Objective To explore the application effect of tourniquet combined with uterine contraction agent in cesarean section of high-risk pu-erpera.Methods 80 high-risk pregnant women who delivered in our hospital from January 2018 to May 2021 were selected as the research subjects,and they were randomly divided into study group and control group,with 40 cases in each group.The control group used the contraction agent carprost tritriol to stop bleeding,and the study group combined tourniquet to stop bleeding on the basis of the control group.The intraoperative blood loss hem-orrhage,hemodynamic indexes,postoperative hemoglobin decline value and milk quality were compared between the two groups.Results The amount of intraoperative blood loss in the study group was lower than that in the control group,and the decrease value of postoperative hemoglobin was lower than that in the control group,the difference was statistically significant(P<0.05).There was no significant difference in heart rate,systol-ic blood pressure,diastolic blood pressure and respiratory rate between two groups.The ratio of lack of milk and insufficient milk in the study group was lower than that in the control group(P<0.05),and the ratio of sufficient milk was higher than that in the control group,the difference was statis-tically significant(P<0.05),there was no statistical significance in the ratio of basic sufficient milk between the two groups.Conclusion Tourniquet combined with uterine contraction agent for postpartum hemostasis during cesarean section can significantly reduce postpartum bleeding,and does not affect maternal hemodynamic indexes and milk quality,which is worthy of clinical application.
