妊娠期糖尿病产妇分娩新生儿发生低血糖的产科因素分析

Obstetric factors of neonatal hypoglycemia in pregnant women with gestational diabetes mellitus

目的分析妊娠期糖尿病产妇分娩新生儿发生低血糖的产科因素。方法回顾性分析2019年3月至2020年3月本院收治的100例妊娠期糖尿病产妇的临床资料,其中血糖正常新生儿作为参照组(n=55),低血糖新生儿作为研究组(n=45)。比较两组临床资料,采用多因素Logistic回归分析妊娠期糖尿病产妇分娩新生儿发生低血糖的产科因素。结果两组产妇孕前超重或肥胖、产妇血糖控制情况、早产新生儿、新生儿低体质量、新生儿低体温、新生儿喂养比较差异有统计学意义(P<0.05)。多因素Logistic分析结果显示,产妇产前超重或肥胖、产妇血糖控制欠佳、新生儿低体温、新生儿低体质量、早产儿、新生儿喂养不当为妊娠期糖尿病产妇分娩新生儿发生低血糖的主要危险因素(P<0.05)。结论妊娠期糖尿病产妇分娩的新生儿可因产妇产前超重或肥胖、产妇血糖控制欠佳等因素而发生低血糖,因此,医护人员需针对上述因素针对性地制订护理措施,以降低新生儿低血糖发生率,进一步提升临床医疗服务质量水平。

Objective To analyze the obstetric factors of neonatal hypoglycemia in pregnant women with gestational diabetes mellitus.Methods The clinical data of 100 cases of pregnant women with gestational diabetes admitted to our hospital from March 2019 to March 2020 were ret-rospectively analyzed,of which the neonates with normal blood sugar were used as the reference group(n=55),and the neonates with hypoglycemia were used as the research group(n=45).The clinical data of the two groups were compared,and the multivariate Logistic regression was used to ana-lyze the obstetric factors of hypoglycemia in neonates of gestational diabetes mellitus mothers.Results There were statistically significant differenc-es in pre-pregnancy overweight or obesity,maternal blood sugar control,premature neonates,neonatal low body weight,neonatal hypothermia,and neonatal feeding between the two groups(P<0.05).The results of multivariate Logistic analysis showed that prenatal overweight or obesity,poor maternal blood glucose control,neonatal hypothermia,neonatal low body weight,preterm infants,and improper neonatal feeding were the main risk factors for neonatal hypoglycemia of gestational diabetes mellitus(P<0.05).Conclusion Neonates delivered by pregnant women with gestational diabetes mellitus may have hypoglycemia due to overweight or obesity before delivery,poor blood glucose control and other factors.Therefore,medi-cal staff should formulate targeted nursing measures for the above factors to reduce the incidence of neonatal hypoglycemia and further improve the quality of clinical medical service.
