马来酸噻吗洛尔滴眼液外用治疗婴幼儿血管瘤疗效的多因素Logistic分析

Multivariate Logistic analysis on the curative effect of topical timolol maleate eye drops on infantile hemangioma

目的探讨马来酸噻吗洛尔滴眼液外用治疗婴幼儿血管瘤疗效的影响因素。方法选取2019年1月至2020年11月于本院确诊的120例血管瘤婴幼儿作为研究对象,随机分为观察组和对照组,每组60例。对照组采用盐酸普萘洛尔片治疗,观察组采用马来酸噻吗洛尔滴眼液外用治疗,比较两组临床疗效、不良反应发生情况。根据临床疗效将观察组分为有效组(优+良,n=42)与无效组(中+差,n=18)。比较两组性别、年龄、瘤体大小、病发部位及血管瘤程度,采用Logistic回归分析影响马来酸噻吗洛尔滴眼液外用治疗婴幼儿血管瘤疗效的危险因素。结果观察组治疗优良率为70.00%,对照组为66.67%,两组比较差异无统计学意义。观察组不良反应发生率为1.67%,明显低于对照组的15.00%,差异有统计学意义(P<0.05)。有效组年龄≤6个月、瘤体<5 cm占比均高于无效组,差异有统计学意义(P<0.05)。年龄>6个月,瘤体≥5 cm为影响马来酸噻吗洛尔滴眼液外用治疗婴幼儿血管瘤疗效的独立危险因素(P<0.05)。结论马来酸噻吗洛尔滴眼液外用治疗婴幼儿血管瘤的疗效显著,患儿年龄越小、瘤体越小,治疗效果越好。

Objective To explore the influencing factors of curative effect of topical timolol maleate eye drops on infantile hemangioma.Methods 120 infants with hemangioma confirmed in the hospital from January 2019 and November 2020 were enrolled as the research subjectss.They were randomly divided into observation group and control group,with 60 cases in each group.The control group was treated with propranolol hydrochloride tablets,while observation group was treated with topical timolol maleate eye drops.The clinical curative effect and occurrence of adverse reactions were compared between the two groups.observation group were divided into effective group(excellent and good effect,n=42)and ineffective group(moderate+poor effect,n=18)according to clinical curative effect.The gender,age,tumor size,onset sites and severity of hemangioma were compared between the two groups,the influencing factors of curative effect of topical timolol maleate eye drops were analyzed by Logistic regression analysis.Results The excellent and good rate in the observation group was 70.00%,while the control group was 66.67%,and there was no significant difference between the two groups.The incidence of adverse reactions in the observation group was 1.67%,which was significantly lower than 15.00%in the control group,and the difference was statistically significant(P<0.05).The age of≤6 months and tumor size<5 cm in the effective group were higher than those in the ineffective group,and the difference was statistically significant(P<0.05).Age>6 months and tumor size≥5 cm were independent risk factors affecting the efficacy of topical timolol maleate eye drops in the treatment of infantile hemangioma(P<0.05).Conclusion The curative effect of topical timolol maleate eye drops is significant on infantile hemangioma,the curative effect is better in infants with younger age and smaller tumor size.
