Flexi-bar运动干预治疗非特异性下腰痛的系统评价与Meta分析

Systematic Review and Meta-analysis of Flexi-bar Exercise Intervention in the Treatment of Nonspecific Low Back Pain

摘要 目的:系统评价Flexi-bar运动干预治疗非特异性下腰痛患者疼痛及腰椎功能障碍的效果。方法:计算机检索PubMed、Embase、Scopus、Web of Science、中国知网、万方、维普等数据库,收集国内外关于Flexi-bar训练对下腰痛患者干预的随机对照实验相关文章。检索时间为从建库至2021年12月9日。两名研究人员对纳入文献信息进行提取。采用TESTEX量表对纳入文献进行质量评价。应用STATA 16.0对纳入文献的结局指标进行分析,效应量采用均数差(WMD)表示。采用敏感性分析评价纳入文章对总体效应的影响。结果:以视觉模拟评分量表(Visual Analogue Scale, VAS)为指标纳入文献5篇,meta分析结果显示Flexi-bar运动干预组较常规治疗组缓解非特异性下腰痛患者疼痛的效果更好(WMD=-1.22, 95%CI:-1.96,-0.48),纳入研究呈中等异质性(I^(2)=66.14%);以腰椎功能障碍指数(Oswestry Disability Index, ODI)为指标纳入文献5篇,meta分析结果显示Flexi-bar运动干预组较常规治疗组改善非特异性下腰痛患者腰椎功能障碍的效果不显著(WMD=-1.88, 95%CI:-4.82,1.06),异质性较高(I^(2)=87.89%)。结论:Flexi-bar运动干预对于缓解非特异性下腰痛患者疼痛的效果比单独进行常规治疗的效果更好。在改善患者的腰椎功能障碍方面,Flexi-bar运动干预的效果不显著。 This paper aims to systematically evaluate the effect of Flexi-bar exercise intervention on pain and lumbar spine dysfunction in patients with nonspecific low back pain. Methods: PubMed, Embase, Scopus, Web of Science, CNKI, Wanfang, VIP and other databases were searched by computer to collect domestic and overseas articles on the intervention of Flexi-bar training on patients with low back pain until December 9 of 2021. Two researchers extracted the information of the included literature. The TESTEX scale was used to evaluate the quality of the included literature. STATA 16.0 was used to analyze the outcome indicators of the included literature, and the effect size was measured by the mean difference(WMD). Sensitivity analysis was used to evaluate the impact of the included articles on the overall effect. Results: The Visual Analogue Scale(VAS)was used as the index to include 5 literatures. The meta-analysis showed that the Flexi-bar exercise therapy group had better pain relief in patients with non-specific low back pain than the conventional treatment group(WMD=-1.22, 95% CI:-1.96,-0.48), the included studies showed moderate heterogeneity(I^(2)=66.14%);5 articles were included with the Oswestry Disability Index(ODI)as the index, and the meta-analysis results showed that compared with the conventional treatment group, the flexi-bar exercise intervention group had no significant effect on improving lumbar spine dysfunction in patients with non-specific low back pain(WMD=-1.88,95%CI:-4.82,1.06), and the heterogeneity was higher(I^(2)=87.89%). Conclusion: Flexi-bar exercise intervention is more effective in relieving pain in patients with non-specific low back pain than conventional treatment alone. Flexi-bar exercise intervention has no significant effect on improving lumbar spine dysfunction in patients.
