摘要
目的 分析磁共振成像(MRI)与超声心动图用于评价扩张型心肌病(DCM)患者左室收缩功能价值。方法 选取2017年1月至2021年6月我院收治的DCM患者96例为观察组。另选取我院同期健康体检者48例为对照组。比较两种方法 检测DCM患者和健康人心脏结构参数,包括左室舒张末期室壁厚度(TDD)、左室收缩末期室壁厚度(TSD)、室间隔基底段厚度(IVS)及左室心肌质量(LVMM)。比较两种不同方法 检测DCM患者左心室功能参数,包括左心室射血分数(LVEF)、左心室收缩末期容积(LVEDV)、舒张末期容积(ESV)、及左心室的每搏输出量(sV)。比较两种方法 对DCM诊断的检出率和诊断符合率。Pearson分析两种方法 检测左心室收缩功能参数相关性。结果 采用两种方法 检测入组患者,观察组患者TDD、TSD低于对照组,LVMM高于对照组(P<0.05)。两组方法 之间TDD、TSD、IVS和LVMM参数之间无显著性差异(t=0.513,t=0.280,t=184,t=0.221,P<0.05)。MRI检查对DCM检出率为100.00%(96/96)、诊断符合率为98.96%(95/96);超声心动图检查对DCM检出率为100.00%(96/96)、诊断符合率为97.91%(94/96)。两种方式对DXM检出率和诊断符合率比较无明显差异(P>0.05)。两种方法 检测DCM患者显示心脏呈球形扩张,心肌功能受损。两种方法 检查DCM患者左心室功能参数,两者LVEDV、LVESV、sV和LVEF参数之间无显著性差异(P>0.0。经相关性分析两组左心室收缩功能参数,LVESV (r=0.79,P<0.05)和LVEF (r=0.77,P<0.05)呈显著相关, LVEDV(r=0.65,P<0.05)和sV(r=0.62,P<0.05)呈中等相关。结论 mRI与超声心动图对DCM患者具有较高的检测率,且两种方法 具有良好的相关性,值得临床推广使用。
Objective To analyze the value of magnetic resonance imaging(MRI) and echocardiogram in evaluating left ventricular systolic function of patients with dilated cardiomyopathy(DCM).Methods A total of96 patients with DCM admitted to the hospital were enrolled as observation group between January2017 and June 2021,while other 48 healthy controls during the same period were enrolled as control group.The cardiac structure parameters in DCM patients and healthy controls detected by the two methods were compared,including left ventricular end diastolic wall thickness(TDD),left ventricular wall thickness at end systole(TSD),thickness of basal segment of ventricular septum(IVS) and left ventricular myocardial mass(LVMM).The left ventricular function parameters in DCM patients detected by the two methods we re compared,including left ventricular ejection fraction(LVEF),left ventricular end-diastolic volume(LVEDV),end-systolic volume(ESV) and left ventricular stroke volume(sV).The detection rate and diagnosis coincidence rate of DCM between the two methods were compared.The correlation of left ventricular systolic function parameters detected by the two methods was analyzed by Pearson.Results According to the two detection methods,TDD and TSD in observation group were lower than those in control group,while LVMM was higher than that in control group(P<0.05).There was no significa nt difference in TDD,TS D,IVS or LVMM between the two methods(t=0.513,t=0.280,t=0.184,t=0.221,P<0.05).The detection rate and diagnosis coincidence rate of DCM by MRI were 100.00%(96/96) and 98.96%(95/96),which by echocardiogram were100.00%(96/96) and 97.91%(94/96).There was no significant difference in the detection rate and diagnosis coincidence rate of DCM between the two methods(P>0.05).The two detection methods showed that there was spherical dilatation of hea rt and myocardial function damage in DCM patients.There was no significant difference in LVEDV,LVESV,sV or LVEF between the two methods(P>0.05).Correlation analysis showed that LVESV(r=0.79,P<0.05) was significa ntly correlated with LVEF(r=0.77,P<0.05),LVEDV(r=0.65,P<0.05) was moderately correlated with sV(r=0.62,P<0.05).Conclusion The detection efficiency of M RI and echoca rdiogram is high for DCM patients.And there is good correlation between the two methods.
