两种腹腔镜下疝修补术治疗腹股沟疝的临床效果探讨

To explore the clinical effects of two kinds of laparoscopic hernia repairs in the treatment of inguinal hernia

目的分析两种腹腔镜下疝修补术治疗腹股沟疝的临床效果。方法回顾性分析兴化市第五人民医院2017年2月—2020年2月收治的成人腹股沟疝患者50例的临床资料,按照不同的腹腔镜下疝修补术方法分为TEP组(n=28)及TAPP组(n=22),TEP组给予腹腔镜下完全腹膜外腹股沟疝修补术,TAPP组给予腹腔镜腹膜前修补术,对比2组围术期临床指标、术后并发症以及术后随访1年期间复发情况。结果2组术中出血量、术后下床时间、住院时间、住院总费用、术后疼痛持续时间均无明显差异(P>0.05),TEP组手术时间短于TAPP组,差异有统计学意义(P<0.05)。2组术后局部血肿、暂时性神经感觉异常、切口感染、尿潴留、阴囊血肿(气肿)等并发症发生率均无明显差异,术后随访1年期间复发率也无明显差异(P>0.05)。结论腹腔镜下完全腹膜外腹股沟疝修补术与腹腔镜腹膜前修补术治疗腹股沟疝均可取得较好的效果,但前者因手术时间相对较短更具临床优势。

Objective To explore and analyze the clinical effects of two laparoscopic hernia repair procedures in the treatment of inguinal hernia.Methods The clinical data of 50 adult inguinal hernia patients admitted to our hospital from February 2017 to February 2020 were retrospectively analyzed.According to different methods of laparoscopic hernia repair,they were divided into TEP group(n=28)and In the TAPP group(n=22),the TEP group was given laparoscopic complete extraperitoneal inguinal hernia repair,and the TAPP group was given laparoscopic preperitoneal repair.The perioperative clinical indicators,postoperative complications and postoperative follow-up were compared between the two groups1 Recurrence during the year.Results Compared with the TAPP group,there was no significant difference in intraoperative blood loss,postoperative time to get out of bed,hospitalization time,total hospitalization expenses,and postoperative pain duration(P>0.05).Compared with the TAPP group,the operation time was shorter in the TEP group,and the difference was statistically significant(P<0.05).There was no significant difference in the incidence of postoperative complications of local hematoma,transient neurosensory abnormalities,incision infection,urinary retention,and scrotal hematoma(emphysema)between the TEP and TAPP groups,and there was no significant difference in the recurrence rate during the 1-year follow-up period.Difference(P>0.05).Conclusion Both laparoscopic total extraperitoneal inguinal hernia repair and laparoscopic preperitoneal repair can achieve good results in the treatment of inguinal hernia,but the former has a relatively short operation time and has more clinical advantages.
