腹膜后穿刺置管引流术治疗重症急性胰腺炎的效果及对患者血清相关指标的影响

Effect of retroperitoneal puncture and drainage in the treatment of severe acute pancreatitis and its effect on serum related indexes

目的 探讨腹膜后穿刺置管引流术(PRCD)治疗重症急性胰腺炎(SAP)的效果及对患者血清C反应蛋白(CRP)、降钙素原(PCT)、血清淀粉酶样蛋白A(SAA)水平的影响。方法 选取2018年5月至2019年9月本院收治的74例重症急性胰腺炎患者作为研究对象,按照随机数字抽取法分为观察组和对照组,各37例。对照组采用腹腔穿刺置管引流术(APD)治疗,观察组采用PRCD治疗,比较两组血清CRP、PCT、SAA水平及恢复情况、并发症情况。结果 治疗后,观察组CRP、PCT、SAA水平均低于对照组,差异有统计学意义(P<0.05);两组症状消失时间、血清淀粉酶恢复时间、引流时间、ICU住院时间、总住院时间比较差异无统计学意义;两组并发症发生率比较差异无统计学意义。结论 PRCD治疗重症急性胰腺炎效果显著,可改善患者生化指标水平和临床症状,且并发症少,值得临床推广应用。

Objective To investigate the effect of retroperitoneal puncture and drainage in the treatment of severe acute pancreatitis(SAP) and its influence on serum C-reactive protein(CRP), procalcitonin(PCT) and serum amyloid A(SAA) levels. Methods 74 patients with severe acute pancreatitis from May 2018 to September 2019 were selected as the research subjects, and they were randomly divided into observation group and control group according to random number sampling method, with 37 cases in each group. The control group was treated with abdominal puncture drainage(APD), and the observation group was treated with retroperitoneal puncture drainage(PRCD). The serum CRP, PCT and SAA levels, recovery and complications between the two groups were compared. Results After treatment, the levels of CRP, PCT and SAA in the observation group were lower than those in the control group(P<0.05);there was no significant difference in symptom disappearance time, serum amylase recovery time, drainage time, ICU hospital stay and total hospital stay between the two groups;there was no significant difference in the incidence of complications between two groups. Conclusion PRCD in the treatment of severe acute pancreatitis has significant effect, can improve biochemical index levels and clinical symptoms, with less complications, which is worthy application.
