摘要
目的探讨微波消融术对甲状腺肿患者甲状腺功能的中远期影响。方法选取2017年12月至2019年10月于本院行微波消融术治疗的46例甲状腺肿患者作为观察组,另选取同期行手术切除的50例甲状腺肿患者作为对照组,比较两组患者术后并发症及术后6、12个月甲状腺功能情况。结果观察组并发症发生率明显低于对照组,差异有统计学意义(P<0.05);观察组术后血清游离三碘甲腺原氨酸(FT_(3))、游离甲状腺素(FT_(4))及促甲状腺激素(TSH)水平均低于术前,对照组各指标均高于术前,但与术前比较差异均无统计学意义;术后6、12个月,观察组FT_(3)、FT_(4)及TSH水平均明显低于对照组,差异有统计学意义(P<0.05)。结论应用微波消融术治疗甲状腺肿并发症少,术后患者甲状腺代谢功能正常,中远期疗效满意,值得临床推广应用。
Objective To investigate the medium and long term effects of microwave ablation on thyroid function in patients with goiter.Methods 46 patients with goiter who underwent microwave ablation in our hospital from December 2017 to October 2019 were selected as the observation group,and 50 patients with goiter who underwent surgical resection at the same time were selected as the control group.The postoperative complications and thyroid function at 6 and 12 months after surgery were compared between the two groups.Results The incidence of complications in the observation group was significantly lower than that in the control group,the difference was statistically significant(P<0.05);the postoperative serum free triiodothyronine(FT_(3)),free thyroxine(FT_(4))and thyroid-stimulating hormone(TSH)indexes in the observation group were lower than those before the operation,while the indexes in the control group were higher than those before surgery,but there was no statistically significant difference compared with before surgery;6 and 12 months after surgery,FT_(3),FT_(4) and TSH in the observation group were significantly lower than those in the control group,the difference was statistically significant(P<0.05).Conclusion There are few clinical complications of microwave ablation in the treatment of goiter,postoperative patients have normal thyroid metabolic function,and the long term efficacy is satisfactory,which is worthy of clinical application.
