腮腺部分切除术治疗腮腺浅叶良性肿瘤的临床疗效观察

Clinical observation of partial parotidectomy in the treatment of benign tumors in the superficial lobe of parotid gland

目的 探讨腮腺部分切除术治疗腮腺浅叶良性肿瘤的临床疗效。方法 选取2012年1月至2020年3月于本院接受治疗的52例腮腺浅叶良性肿瘤患者作为研究对象,根据随机数字表法分为对照组和观察组,每组26例。对照组行腮腺浅叶腺体全切术,观察组行腮腺部分切除术,比较两组治疗前后生物学指标、手术指标及复发率和并发症发生情况。结果 治疗后,观察组β-连环蛋白、G1/S-特异性周期蛋白-D1(Cyclin-D1)和热休克蛋白70(HSPs70)水平低于对照组(P<0.05);细胞周期蛋白(P27)水平高于对照组(P<0.05);两组手术时间比较差异无统计学意义;观察组术中出血量及神经分支数均少于对照组,切口长度短于对照组(P<0.05);术后6个月,两组复发率比较差异无统计学意义;术后,观察组并发症发生率低于对照组(P<0.05)。结论 腮腺部分切除术治疗腮腺浅叶良性肿瘤中效果显著,能明显改善患者生物学指标,创伤小,术中出血量少,并发症发生率低,值得临床推广应用。

Objective To investigate the clinical effect of partial parotidectomy in the treatment of benign tumors in the superficial lobe of parotid gland. Methods 52 patients with benign tumors in the superficial lobe of parotid gland treated in our hospital from January 2012 to March 2020were selected as the research subjects, and they were divided into control group and observation group according to random number table method,with 26 cases in each group. The control group was treated with total parotidectomy, and the observation group was treated with partial parotidectomy, the biological indexes, surgical indexes, recurrence rate and complications were compared between the two groups before and after treatment.Results After treatment, the levels of β-catenin, G1/s-specific cyclin-D1(Cyclin-D1) and heatshockprotein70(HSPs70) in the observation group were lower than those in the control group, and the level of cyclinprotein(P27) was higher than that in the control group(P<0.05);There was no significant difference in operation time between the two groups;the amount of bleeding and the number of nerve branches in the observation group were less than those in the control group, and the length of incision was shorter than that in the control group(P<0.05);6 months after operation, there was no significant difference in the recurrence rate between the two groups;after operation, the incidence of complications in the observation group was lower than that in the control group(P<0.05). Conclusion Partial parotidectomy is effective in the treatment of benign tumors in the superficial lobe of parotid gland, it can significantly improve the biological indexes of patients, with less trauma, less intraoperative bleeding and low incidence of complications, it is worthy of clinical application.
