甲状腺次全切除术治疗双侧结节性甲状腺肿患者的疗效分析

Analysis of the curative effect of subtotal thyroidectomy in the treatment of patients with bilateral nodular goiter

目的 分析甲状腺次全切除术对双侧结节性甲状腺肿患者的治疗效果。方法 回顾性分析2019年4月至2020年4月于本院就诊的68例双侧结节性甲状腺肿患者的临床资料,行甲状腺全切除术的患者作为对照组,行次全切除术的患者作为观察组,每组34例。比较两组手术时间、镇痛剂应用量、手术出血量、住院时间、临床疗效及并发症发生率。结果 观察组手术时间及住院时间分别为(87.35±5.26)min、(6.71±1.22)d,均短于对照组的(114.32±8.67)min、(9.38±1.89)d,差异有统计学意义(P<0.05),镇痛剂应用量、手术出血量分别为(82.30±4.19)mg、(91.47±5.21)ml,均少于对照组的(148.31±5.46)mg、(127.36±7.83)ml,差异有统计学意义(P<0.05);观察组手术后并发症发生率为2.94%,低于对照组的17.64%(P<0.05);两组治疗总有效率比较差异无统计学意义。结论 甲状腺次全切除术治疗双侧结节性甲状腺肿患者疗效显著,可有效减少手术创伤,改善预后,降低术后并发症发生风险,值得临床推广应用。

Objective To analyze the therapeutic effect of subtotal thyroidectomy on patients with bilateral nodular goiter. Methods A retrospective analysis of the clinical data of 68 patients with bilateral nodular goiter who visited our hospital from April 2019 to April 2020, patients who underwent total thyroidectomy were selected as the control group, and patients who underwent subtotal thyroidectomy were selected as the observation group, with 34 cases in each group. The operation time, the amount of analgesic used, the amount of blood loss, the time of hospital stay, the curative effect, and the incidence of complications were compared between the two groups. Results The operation time and the time of hospital stay in the observation group were(87.35±5.26) min and(6.71±1.22) d, respectively, which were shorter than(114.32±8.67) min and(9.38±1.89) d in the control group, the difference was statistically significant(P<0.05). The amount of analgesics used and surgical blood loss in the observation group were(82.30±4.19) mg and(91.47±5.21) ml, respectively, which were lower than(148.31±5.46) mg and(127.36±7.83) ml in the control group, the difference was statistically significant(P<0.05). The incidence of postoperative complications in the observation group was 5.88%, which was lower than23.52% in the control group(P<0.05);there was no significant difference in the total effective rate between the two groups. Conclusion Subtotal thyroidectomy in the treatment of patients with bilateral nodular goiter has significant curative effect, can effectively reduce surgical trauma, improve prognosis, and reduce the risk of postoperative complications, which is worthy of clinical application.
