中药熏洗配合关节内注射透明质酸钠治疗骨性关节炎的临床疗效及安全性

Clinical efficacy and safety of Chinese herbal fumigation combined with intra-articular injection of sodium hyaluronate in the treatment of osteoarthritis

目的 探讨采用中药熏洗配合关节内注射透明质酸钠治疗骨性关节炎患者的临床疗效及安全性。方法 选取2018年1月至2020年12月本院收治的70例骨性关节炎患者作为研究对象,随机分为单一组(关节内注射透明质酸钠治疗)和联合组(中药熏洗配合关节内注射透明质酸钠治疗),各35例。比较两组临床疗效、生命质量评分、Harris量表评分、抑郁自评量表(SDS)评分、疼痛评分及安全性。结果 联合组治疗总有效率为94.29%,高于单一组的71.43%,差异有统计学意义(P<0.05);治疗前,两组生命质量评分、Harris量表评分、SDS量表评分、疼痛评分比较差异无统计学意义;治疗后,联合组生命质量评分[(90.15±2.88)分]、Harris量表评分[(85.23±2.45)分]均高于单一组[(78.88±3.15)、(75.12±5.17)分],SDS量表评分[(25.22±5.29)分]、疼痛评分[(1.22±0.25)分]低于单一组[(40.22±3.12)、(3.59±0.27)分],差异有统计学意义(P<0.05)。两组均未出现严重不良反应,治疗安全性较高。结论 中药熏洗配合关节内注射透明质酸钠治疗骨性关节炎患者效果显著,可提高患者生命质量评分,缓解疼痛程度,且安全性较高,可提高预后水平。

Objective To investigate the clinical efficacy and safety of Chinese herbal fumigation combined with intra-articular injection of sodium hyaluronate in the treatment of patients with osteoarthritis. Methods 70 patients with osteoarthritis treated in our hospital from January 2018 to December 2020 were selected as the research subjects, and they were randomly divided into single group(intra-articular injection of sodium hyaluronate) and combined group(traditional Chinese medicine fumigation combined with intra-articular injection of sodium hyaluronate), with 35 cases in each group. Treatment effect, quality of life score, Harris scale score, self-rating depression scale(SDS) score, pain score and safety were compared between the two groups. Results The total effective rate of combined group was 94.29%, which was higher than 71.43% of single group, the difference was statistically significant(P<0.05). Before treatment, there were no significant differences in quality of life score, Harris scale score, SDS scale score and pain score between the two groups;after treatment, the quality of life scores([90.15±2.88] scores) and Harris scale scores([85.23±2.45]scores) in the combined group were higher than([78.88±3.15], [75.12±5.17] scores) of the single group, SDS score([25.22±5.29] scores) and pain score([1.22±0.25] scores) of the combined group were lower than([40.22±3.12], [3.59±0.27] scores) of single group, the difference was statistically significant(P<0.05). There were no serious adverse reactions in the two groups, and the treatment safety was high. Conclusion Chinese herbal fumigation combined with intra-articular injection of sodium hyaluronate has significant effect on the treatment of patients with osteoarthritis, which can improve the quality of life and reduce pain, with high safety and improved prognosis.
