腹腔镜子宫肌瘤剔除手术和经腹子宫肌瘤剔除手术治疗子宫肌瘤的疗效比较

Comparison of the therapeutic effect of laparoscopic myomectomy and transabdominal myomectomy on uterine fibroids

目的 比较腹腔镜子宫肌瘤剔除手术和经腹子宫肌瘤剔除手术治疗子宫肌瘤的疗效。方法 回顾性分析2019年10月至2020年10月于本院查后确诊的80例子宫肌瘤患者的临床资料,按照手术方式不同分为对照组和研究组,每组40例。对照组采用经腹子宫肌瘤剔除手术治疗,研究组采用腹腔镜子宫肌瘤剔除手术治疗,比较两组手术前后白细胞(WBC)、肿瘤坏死因子(TNF)水平、术中出血量、住院时间及手术后并发症(切口感染、肠梗阻、盆腔粘连)发生情况。结果 术后,对照组WBC、TNF水平均高于研究组(P<0.05);研究组术中出血量为(112.00±10.47)ml,少于对照组的(120.47±11.88)ml,差异有统计学意义(P<0.05);研究组住院时间为(6.60±1.20)d;短于对照组的(7.58±1.41)d;研究组并发症发生率为2.50%,低于对照组的15.00%,差异有统计学意义(P<0.05)。结论 腹腔镜子宫肌瘤剔除手术可降低子宫肌瘤患者体内WBC、TNF水平,减少术中出血量,缩短住院时间,降低患者切口感染、肠梗阻、盆腔黏连的发生率,疗效显著,值得临床推广应用。

Objective To compare and analyze the therapeutic effects of laparoscopic myomectomy and transabdominal myomectomy for uterine fibroids. Methods The clinical data of 80 patients with uterine fibroids diagnosed in our hospital from October 2019 to October 2020 were retrospectively analyzed, and they were divided into control group and study group according to different surgical methods, with 40 cases in each group.The control group was treated with transabdominal myomectomy, and the study group was treated with laparoscopic myomectomy, the levels of white blood cells(WBC) and tumor necrosis factor(TNF) before and after operation, intraoperative blood loss, hospital stay, and complications after surgery(incision infection, intestinal obstruction, pelvic adhesion) were compared between two groups. Results After operation, WBC and TNF levels in control group were higher than those in study group(P<0.05);the amount of intraoperative blood loss in the study group was(112.00±10.47) ml, which was less than(120.47±11.88) ml in the control group, and the difference was statistically significant(P<0.05);the length of hospital stay in the study group was(6.60±1.20) d, which was shorter than(7.58±1.41) d of the control group, and the total incidence of complications in the study group was2.50%, lower than 15.00% in the control group, and the difference was statistically significant(P<0.05). Conclusion Laparoscopic myomectomy can reduce WBC and TNF levels, reduce intraoperative blood loss, shorten hospital stay, and reduce the incidence of incision infection, intestinal obstruction and pelvic adhesion in patients with uterine myoma, the therapeutic effect is obvious, and it is worthy of clinical application.
