椎管内麻醉联合全身麻醉对高龄股骨粗隆间骨折患者麻醉效果及术后恢复的影响

Effect of intraspinal anesthesia combined with general anesthesia on anesthesia effect and postoperative recovery of elderly patients with femoral intertrochanteric fracture

目的 探讨椎管内麻醉联合全身麻醉对高龄股骨粗隆间骨折患者麻醉效果及术后恢复的影响。方法 选取2018年1月至2020年1月于本院进行手术治疗的196例高龄股骨粗隆间骨折患者作为研究对象,按随机数字表法分为对照组(n=96)与观察组(n=100)。对照组实施全身麻醉,观察组在对照组基础上实施椎管内麻醉,比较两组麻醉优良率、手术时间、术中出血量及术后并发症发生率。结果 观察组麻醉优良率明显高于对照组,手术时间明显短于对照组,术中出血量明显少于对照组,术后并发症发生率明显低于对照组,差异均有统计学意义(P<0.05)。结论 采用椎管内麻醉联合全身麻醉的方式对高龄股骨粗隆间骨折患者进行手术麻醉效果显著,可减少术中出血量,有利于缩短手术时间,降低患者并发症发生率,值得临床推广应用。

Objective To investigate the effect of intraspinal anesthesia combined with general anesthesia on anesthesia and postoperative recovery in elderly patients with femoral intertrochanteric fracture. Methods 196 elderly patients with femoral intertrochanteric fracture who underwent surgical treatment in our hospital from January 2018 to January 2020 were selected as the research subjects, and they were divided into control group(n=96) and observation group(n=100) according to random number table method. General anesthesia was performed in the control group and intraspinal anesthesia was performed in the observation group on the basis of the control group. The excellent and good rate of anesthesia, operation time,intraoperative bleeding and the incidence of postoperative complications were compared between the two groups. Results The excellent and good rate of anesthesia in the observation group was significantly higher than that in the control group, the operation time was significantly shorter than that in the control group, the amount of intraoperative bleeding was significantly less than that in the control group, and the incidence of postoperative complications was significantly lower than that in the control group, and the difference was statistically significant(P<0.05). Conclusion Intraspinal anesthesia combined with general anesthesia is effective in the surgical anesthesia of elderly patients with femoral intertrochanteric fracture,which can reduce the amount of intraoperative bleeding, shorten the operation time and reduce the incidence of complications, it is worthy of clinical application.
