不孕症患者冷冻胚胎移植后妊娠结局的影响因素

Influencing factors of pregnancy outcome after frozen embryo transfer in infertile patients

目的 分析不孕症患者冷冻胚胎移植后妊娠结局的影响因素。方法 回顾性分析2019年3月至2020年3月本院收治的60例行冷冻胚胎移植的不孕症患者临床资料,将临床妊娠患者纳入妊娠结局良好组,流产患者纳入妊娠结局不良组。统计所有患者临床资料,分析不孕症患者冷冻胚胎移植后妊娠结局的影响因素。结果 60例不孕症患者经冷冻胚胎移植后,妊娠结局良好39例,占比65.00%;妊娠结局不良21例,占比35.00%。两组子宫内膜厚度、血清维生素D(VitD)、抗苗勒管激素(AMH)、同型半胱氨酸(Hcy)水平比较差异有统计学意义(P<0.05);两组其他临床资料比较差异无统计学意义。经Logistic回归分析显示,子宫内膜厚度小、血清VitD、AMH水平低,Hcy水平高是不孕症患者冷冻胚胎移植后妊娠结局不良的影响因素(OR>1,P<0.05)。结论 子宫内膜厚度小、血清VitD、AMH水平低,Hcy水平高是不孕症患者冷冻胚胎移植后妊娠结局不良的影响因素。

Objective To analyze the influencing factors of pregnancy outcome after frozen embryo transfer in infertile patients. Methods The clinical data of 60 patients with infertility who underwent frozen embryo transfer in our hospital from March 2019 to March 2020 were retrospectively analyzed. Clinical pregnancy patients were included in the good pregnancy outcome group, and miscarriage patients were included in the poor pregnancy outcome group. The clinical data of all patients were collected, and the influencing factors of pregnancy outcomes after frozen embryo transfer in infertile patients were analyzed. Results After frozen embryo transfer in 60 infertile patients, 39 cases had good pregnancy outcomes, accounting for 65.00%;21 cases had poor pregnancy outcomes, accounting for 35.00%. There were significant differences in endometrial thickness, serum vitamin D(VitD), anti-Müllerian hormone(AMH) and homocysteine(Hcy) between the two groups(P<0.05);there was no significant difference in other clinical data between the two groups. Logistic regression analysis showed that small endometrial thickness, low serum VitD and AMH levels, and high Hcy levels were the influencing factors of poor pregnancy outcomes after frozen embryo transfer in infertile patients(OR>1, P<0.05). Conclusion Small endometrial thickness, low serum VitD and AMH levels, and high Hcy levels are the influencing factors of poor pregnancy outcomes after frozen embryo transfer in infertile patients.
