血清超敏C反应蛋白及胱抑素C在小儿过敏性紫癜早期肾损伤检测中的应用价值 被引量:2

摘要 目的探讨血清超敏C反应蛋白(hs-CRP)及胱抑素C(CysC)在小儿过敏性紫癜(HSP)早期肾损伤中的应用价值。方法选取2018年6月至2020年6月本院儿科确诊的90例HSP患儿作为研究组,根据尿微量白蛋白排泄率(UAER)的不同分为HSP不伴肾损伤组(UAER<30 mg/24 h)及HSP合并肾损伤组(30 mg/24 h≤UAER≤300 mg/24 h),各45例。另选同期本院45名健康体检小儿作为对照组。比较各组血清hs-CRP、CysC水平及UAER。结果HSP合并肾损伤组和HSP不伴肾损伤组血清hs-CRP、CysC水平均高于对照组,且HSP合并肾损伤组高于HSP不伴肾损伤组,差异有统计学意义(P<0.05);HSP合并肾损伤组和HSP不伴肾损伤组血清hs-CRP和CysC联合检测的阳性率显著高于对照组,且HSP合并肾损伤组高于HSP不伴肾损伤组,差异有统计学意义(P<0.05)。结论在小儿HSP早期肾损伤的诊断中,血清hs-CRP和CysC联合检测可提高其有效检出率,具有重要临床意义。 Objective To investigate the application value of serum high-sensitivity C-reactive protein(hs-CRP)and cystatin C(CysC)in early renal injury of children with henoch schonlein purpura(HSP).Methods 90 children with HSP diagnosed in pediatrics of our hospital from June 2018 to June 2020 were selected as the study group,according to the urinary microalbumin excretion rate(UAER),they were divided into HSP without renal injury group(UAER<30 mg/24 h)and HSP combined with renal injury group(30 mg/24 h≤UAER≤300 mg/24 h),with 45 cases in each group.Another 45 healthy children in our hospital in the same period were selected as the control group.The levels of serum hs-CRP,CysC and UAER were detected and compared.Results The levels of serum hs-CRP and CysC in HSP combined with renal injury group and HSP without renal injury group were significantly higher than those in control group,and the level of serum hs-CRP and CysC in HSP combined with renal injury group was higher than that in HSP without renal injury group(P<0.05);the positive rate of serum hs-CRP and CysC in HSP combined with renal injury group and HSP without renal injury group was significantly higher than that in the control group,and the HSP combined with renal injury group was higher than that in HSP without renal injury group(P<0.05).Conclusion Diagnosis of early renal injury in children with HSP,the combined detection of serum hs-CRP and CysC can increase the effective detection rate and has important clinical significance.
