中药灌肠治疗慢性前列腺炎用药规律数据挖掘的文献分析

Data Mining of the Medication Rules of Chinese Medicine Enema for the Treatment of Chronic Prostatitis:A Literature Analysis

【目的】运用数据挖掘方法,分析近年来临床研究文献中采用中药灌肠治疗慢性前列腺炎的用药规律。【方法】计算机检索中国知网(CNKI)、万方数据库(Wanfang Data)、维普中文科技期刊数据库(VIP)中所收录的中药灌肠治疗慢性前列腺炎的临床研究文献,根据纳入与排除标准筛选中药灌肠治疗慢性前列腺炎的处方,运用Excel 2016进行频数统计,以SPSS Modeler 18.0软件中的Apriori算法进行关联规则分析,采用SPSS Statistics 23.0软件进行聚类分析。【结果】共纳入处方61首,中药115味。用药频次≥5次的中药有36味,其中用药频次居前5位的药物为黄柏、败酱草、蒲公英、丹参、王不留行;药物功效分类以清热药和活血化瘀药为主;治疗慢性前列腺炎所用中药的性味以性寒味苦为主,归经多属肝经、胃经、心经。关联规则分析得到25个药物组合,支持度最高的药物组合为黄柏→败酱草,置信度最高的药物组合为败酱草→白花蛇舌草、黄柏→土茯苓+败酱草;聚类分析得到7个核心药物组合。【结论】中药灌肠治疗慢性前列腺炎以清热利湿、活血化瘀为主,辅以补肾健脾、疏肝理气;挖掘得到的用药规律契合其病机特点,对治疗慢性前列腺炎的新药开发和新方组合具有一定的参考意义。

Objective To analyze the medication rules for chronic prostatitis treated with Chinese medicine enema in the clinical research literature in recent years by data mining method.Methods The clinical research literature on Chinese medicine enema for chronic prostatitis was retrieved from the databases of CNKI,Wanfang Data and VIP by computer,and the prescriptions of Chinese medicine enema for chronic prostatitis were screened according to inclusion and exclusion criteria.Excel 2016 was used for frequency statistics,the Apriori algorithm in SPSS Modeler 18.0 software was used for association rule analysis,and SPSS Statistics 23.0 software was used for clustering analysis.Results A total of 61 prescriptions and 115 herbs were included.There were 36 herbs with the medication frequency≥5,among which the top 5 herbs were Cortex Phellodendri,Herba Patriniae,Herba Taraxaci,Radix Salviae Miltiorrhizae and Semen Vaccariae.The herbs were mainly classified into heat-clearing herbs and blood-activating and stasis-removing herbs according their efficacy.The property of Chinese herbs used for chronic prostatitis is characterized by being cold and bitter,and the herbs usually have the meridian tropism of liver,stomach and heart meridians.Twenty-five herbal combinations were obtained after association rule analysis.The herbal combination with the highest support degree was Cortex Phellodendri→Herba Patriniae,and the herbal combinations with the highest confidence were Herba Patriniae→Herba Hedyotis,Cortex Phellodendri→Rhizoma Smilacis Glabrae+Herba Patriniae.Seven core herbal combinations were obtained after cluster analysis.Conclusion The treatment of chronic prostatitis with Chinese medicine enema is based on the therapy of clearing heat and draining dampness,and activating blood and resolving stasis,and can be supplemented by the therapy of tonifying kidney and strengthening spleen,and soothing liver and regulating qi.The medication rules of Chinese medicine enema accords with the characteristics of its pathogenesis and has certain reference significance for the development of new medicines and new combinations of formula for chronic prostatitis.
