血清丝氨酸蛋白酶抑制剂B1 纤维胶凝蛋白-3及亲吻素水平与妊娠期糖尿病的相关性分析

Analysis on the correlations between serum SerpinB1,Ficolin-3,and Kisspeptin levels and gestational diabetes mellitus

目的 探讨血清丝氨酸蛋白酶抑制剂B1(SerpinB1)、纤维胶凝蛋白-3(Ficolin-3)及亲吻素(Kisspeptin)水平与妊娠期糖尿病(GDM)的相关性。方法 选取2018年4月—2020年4月天津市津南医院、天津医科大学第二医院收治的GDM患者92例为研究对象并纳入GDM组,另选同期在该院进行产前检查的健康孕妇50例为对照组。对GDM组患者和对照组受试者的血清SerpinB1、Ficolin-3及Kisspeptin水平及血糖指标、胰岛素抵抗指标进行检测对比,分析探讨血清SerpinB1、Ficolin-3及Kisspeptin水平与GDM的相关性,并采用logistic多因素回归分析对GDM的影响因素进行分析。结果 GDM组患者的SerpinB1水平低于对照组,Ficolin-3、Kisspeptin水平高于对照组,差异有统计学意义(P<0.05)。GDM组患者的空腹血糖(FPG)、餐后2 h血糖(2hPG)、糖化血红蛋白(HbA1c)、空腹胰岛素(FINS)、胰岛素抵抗指数(HOMA-IR)水平均高于对照组,差异有统计学意义(P<0.05)。经Pearson检验分析,GDM患者血清SerpinB1与FPG、2hPG、HbA1c、FINS及HOMA-IR呈负相关性,而Ficolin-3、Kisspeptin与上述指标呈正相关性(P<0.05)。logistic多因素回归分析显示,血清SerpinB1是GDM的保护因素,而Ficolin-3、Kisspeptin是GDM发生的危险因素。结论 GDM人群中SerpinB1呈现低表达,Ficolin-3、Kisspeptin呈现高表达,且与血糖和胰岛素抵抗指标有密切相关性,通过对孕妇SerpinB1、Ficolin-3及Kisspeptin水平的检测,能为GDM的预防、诊断及治疗提供相关的参考依据。

Objective To explore the correlations between serum SerpinB1,Ficolin-3,and Kisspeptin levels and gestational diabetes mellitus(GDM).Methods From April 2018 to April 2020,92 GDM patients treated in Tianjin Jinnan Hospital and the Second Hospital of Tianjin Medical University were selected as GDM group,50 healthy pregnant women receiving prenatal examination were selected as control group.The levels of serum SerpinB1,Ficolin-3,Kisspeptin,blood glucose indicators,and insulin resistance indicators in GDM group and control group were detected and compared.The correlations between serum SerpinB1,Ficolin-3,and Kisspeptin levels and GDM were analyzed.Multivariate logistic regression analysis was used to analyze the influencing factors of GDM.Results SerpinB1 level in GDM group was lower than that in control group,the levels of Ficolin-3 and Kisspeptin were higher than those in control group,there were statistically significant differences(P<0.05).The levels of fasting plasma glucose(FPG),2-hour postprandial blood glucose(2 hPG),glycosylated hemoglobin(HbA1 c),fasting insulin(FINS),and insulin resistance index(HOMA-IR) in GDM group were statistically significantly higher than those in control group(P<0.05).Pearson test showed that SerpinB1 was negatively correlated with FPG,2 hPG,HbA1 c,FINS,and HOMA-IR,while Ficolin-3 and Kisspeptin were positively correlated with FPG,2 hPG,HbA1 c,FINS,and HOMA-IR(P<0.05).Multivariate logistic regression analysis showed that SerpinB1 was the protective factor of GDM,while Ficolin-3 and Kisspeptin were the risk factors of GDM.Conclusion In GDM population,SerpinB1 expression is low,Ficolin-3 and Kisspeptin expressions are high,which are closely related to blood glucose and insulin resistance indexes.The detection of SerpinB1,Ficolin-3,and Kisspeptin levels in pregnant women can provide reference basis for prevention,diagnosis,and treatment of GDM.
