缺氧诱导因子-1α 乳腺癌易感基因1及细胞增殖核抗原在子宫内膜癌中的表达及其与患者预后的相关性 被引量:3

摘要 目的 探讨缺氧诱导因子-1α(HIF-1α)、乳腺癌易感基因1(BRCA1)及细胞增殖核抗原(Ki-67)在子宫内膜癌中的表达及其与患者预后的相关性。方法 选取2018年3月—2020年3月乐清市开发区同乐医院和温州市中心医院收治的62例子宫内膜癌患者为子宫内膜癌组,选取同期子宫内膜增生患者60例为子宫内膜增生组,另选取同期参加体检的长期月经失调患者63例为对照组。检测HIF-1α、BRCA1及Ki-67在子宫内膜癌中的表达,并分析其与子宫内膜癌患者不良预后的相关性。结果 子宫内膜癌组、子宫内膜增生组患者的HIF-1α[(9.27±1.46)、(7.29±1.33)]、Ki-67[(8.36±2.04)、(2.07±0.96)]表达高于对照组[(3.71±0.28)、(1.05±0.13)],差异均有统计学意义(子宫内膜癌组t=29.679、28.335,子宫内膜增生组t=20.888、8.335,均P<0.05);且子宫内膜癌组高于子宫内膜增生组,差异均有统计学意义(t=7.823、21.672,均P<0.05)。子宫内膜癌组、子宫内膜增生组患者的BRCA1[(1.24±0.11)、(3.25±0.74)]表达低于对照组(8.71±2.71),差异均有统计学意义(t=-27.070、-18.490,均P<0.05);且子宫内膜癌组低于子宫内膜增生组,差异有统计学意义(t=-21.818,P<0.05)。HIF-1α表达与子宫内膜癌FIGO分期、肿瘤组织学分级、肌层浸润深度及淋巴结转移均相关,差异均有统计学意义(均P<0.05);BRCA1、Ki-67表达与子宫内膜癌FIGO分期、肿瘤组织学分级及淋巴结转移有相关性,差异均有统计学意义(均P<0.05);但BRCA1、Ki-67表达与子宫内膜癌肌层浸润深度均无相关性,差异均无统计学意义(均P>0.05)。子宫内膜细胞组织中BRCA1表达和子宫内膜癌患者的不良预后呈负相关关系(r=-0.548,P<0.05);而子宫内膜细胞组织中HIF-1α、Ki-67表达和子宫内膜癌患者的不良预后呈正相关关系(r=0.861、0.682,均P<0.05)。结论 HIF-1α、BRCA1及Ki-67蛋白表达和子宫内膜癌患者的病理特征、预后具有相关性,可为子宫内膜癌发病、进展及预后评估提供依据。 Objective To explore the expressions of hypoxia-inducible factor-1α(HIF-1α),breast cancer susceptibility gene 1(BRCA1),and proliferating cell nuclear antigen(Ki-67) in endometrial cancer and the correlations with prognosis of the patients.Methods From March 2018 to March 2020,62 patients with endometrial cancer treated in Tongle Hospital of Yueqing City Development Area and Wenzhou Central Hospital were selected as endometrial cancer group,60 patients with endometrial hyperplasia were selected as endometrial hyperplasia group,63 patients with menstrual disorder for a long time who received physical examination were selected as control group.The expressions of HIF-1α,BRCA1,and Ki-67 in endometrial cancer were detected,the correlations with adverse prognosis of patients with endometrial cancer were analyzed.Results The expression levels of HIF-1 α and Ki-67 in endometrial cancer group and endometrial hyperplasia group were(9.27±1.46),(7.29±1.33) and(8.36±2.04),(2.07±0.96),respectively,which were statistically significantly higher than those in control group [(3.71±0.28),(1.05±0.13)](endometrial cancer group t=29.679,28.335,endometrial hyperplasia group t=20.888,8.335,all P0.05).In endometrial cancer,BRCA1 expression was negatively correlated with adverse prognosis of patients with endometrial cancer(r=-0.548,P<0.05),the expressions of HIF-1α and Ki-67 were positively correlated with adverse prognosis of patients with endometrial cancer(r=0.861,0.682,P<0.05).Conclusion The expressions of HIF-1α,BRCA1,and Ki-67 are correlated with pathological characteristics and prognosis of patients with endometrial cancer,which can provide a basis for occurrence,development,and prognosis of endometrial cancer.
