蜜煎导法治疗老年危重患者便秘的疗效观察

Clinical observation of honey suppository in the treatment of constipation in elderly critically ill patients

目的 观察蜜煎导法治疗老年危重患者便秘的临床效果。方法 选取80例南京市溧水区中医院急诊病房住院的危重且出现便秘的老年患者,按照随机数字表法分为对照组(40例)和观察组(40例)。对照组给予开塞露40 mL纳肛治疗,每日1次,连续治疗14 d;观察组给予蜜煎导纳肛治疗,每日1次,连续治疗14 d。比较两组的临床治疗有效率、便秘症状严重度评分(SS)、Bristol大便性状评分、首次排便花费时间、首次排便时间、治疗期间并发症(尿失禁、痔疮、大肠穿孔)发生情况。结果 观察组临床治疗有效率为97.50%(39/40),高于对照组(82.50%,33/40)(P=0.025)。治疗后,观察组SS评分低于对照组,Bristol大便性状评分高于对照组,首次排便花费时间、首次排便时间均短于对照组(P均<0.05)。治疗过程中两组均未发生严重并发症。结论 蜜煎导法可明显提高老年危重患者便秘的临床有效率,有效改善便秘严重程度。

Objective To observe the clinical effect of honey suppository in the treatment of constipation in elderly critically ill patients.Methods A total of 80 elderly patients with severe constipation who were hospitalized in the emergency ward of Nanjing Lishui District Hospital of Traditional Chinese Medicine were selected and divided into control group(40 cases)and observation group(40 cases)according to random number table method.The control group was treated with 40 mL glycerine enema,once a day,for 14 days.The observation group received promoting defecation with honey suppository,once a day,for 14 days.The clinical treatment efficiency,constipation symptom severity score(SS),Bristol stool trait score,the time it took to pass the first bowel movement,the time of first defecation,and complications(urinary incontinence,hemorrhoids,colon perforation)during treatment were compared between the two groups.Results The effective rate of the observation group was 97.50%(39/40)higher than that of the control group(82.50%,33/40)(P=0.025).After treatment,SS score of the observation group was significantly lower than that of the control group,Bristol stool trait score was significantly higher than that of the control group,and the time it took to pass the first bowel movement and the time of first defecation were shorter than that of the control group(all P<0.05).No serious complications occurred during the treatment in the two groups.Conclusion Promoting defecation with honey suppository can significantly improve the clinical efficiency and severity of constipation in elderly patients with severe constipation.
