基于数据挖掘的中医药治疗社区获得性肺炎组方用药规律研究

Research on the Prescription Rules of Traditional Chinese Medicine in the Treatment of Community-acquired Pneumonia Based on Data Mining

目的总结中医药治疗社区获得性肺炎(CAP)的中药配伍及组方用药规律。方法检索中国知网、万方数据库、维普期刊数据库2005年1月1日—2020年12月31日关于中药复方治疗CAP的临床研究文献,统计药物使用及性味归经分布情况,对高频药物进行二项关联、三项关联规则及系统聚类。结果纳入文献111篇,筛选方剂111首,涉及药物124味,药物类别15种。用药频次≥10次的中药共计28味,其中使用频次前3位的分别为甘草(88)、杏仁(71)、黄芩(68);药物类别出现频次前2位分别为清热药(32)、化痰止咳平喘药(23)。用药频次≥2的中药共计95味,频次前3位的药物药性分别为寒(29)、温(21)、微寒(17),药味分别为苦(44)、甘(41)、辛(38),归经分别为肺(57)、胃(42)、脾(31)。用药频次≥10次的28味药物中二项关联共提取12对,三项关联共7组,系统聚类聚为6类时效果较好。结论中医药治疗CAP以苦甘辛、归肺经药物居多,组方思路以解表清肺化痰、降气止咳平喘为主,兼顾滋养肺阴。

Objective To summarize the compatibility and prescription rules of traditional Chinese medicine in the treatment of communityacquired pneumonia(CAP).Methods The clinical research literatures of CAP with Chinese medicine compound treatment from January 1,2005 to December 31,2020 were retrieved from Chinese National Knowledge Infrastructure(CNKI),Wanfang Database and VIP Journal Database.Drug use,the distribution of nature,flavor,and channel tropism were counted and high-frequency drugs were analyzed by binomial and tertiary correlation analysis and cluster analysis.Results A total of 111 papers were included in the literature,and 111prescriptions were screened,involving 124 drugs and 15 drug classes.There are a total of 28 kinds Chinese medicines with a frequency≥10times,of which the top 3 frequencies were Radix Glycyrrhizae(88),Semen Armeniaccae Amarum and Radix Scutellariae(68).The top 2drug categories were heat-clearing drugs(32),dissipating phlegm,reliving cough and anti-asthmatic drugs(23).There were a total of 95kinds Chinese medicine with a frequency of≥2,and the top 3 drug natures were cold(29),warm(21),and slightly cold(17),and the top 3 flavors were bitter(44),sweet(41),and pungent(38).The channel tropism were the lung(57),stomach(42)and spleen(31).Among the 28 drugs with frequency≥10 times,there were 12 pairs of two associations,and the three associations totaled 7 groups.When clustered into 6 categories,the clustering effect was better.Conclusion The treatment drug of CAP was characterized by bitter sweet,and pungent in nature and channel tropism of the lung.The prescriptions ideas are mainly to resolve exterior,clear the lung heat,dissipate phlegm,relive cough and antiasthmatic,while covering nourishing the lung yin.
