内热针联合复元活血汤治疗腰椎间盘突出症临床观察

Clinical Observation on the Clinical Effect of Internal Heat Needle Combined with Fuyuan Huoxue Decoction in the Treatment of Lumbar Disc Herniation

目的考察内热针联合复元活血汤治疗腰椎间盘突出症的临床疗效。方法选取2017年9月—2020年9月收治的腰椎间盘突出症患者120例,随机将患者分为针药联合组(予以内热针联合复元活血汤治疗)、内热针组(单独予以内热针治疗)以及中药组(单独予以复元活血汤治疗),各40例,连续治疗2~3个疗程。考察疗程结束后7 d、1个月以及3个月腰椎评分(JOA)、视觉模拟评分(VAS)、腰屈曲范围(LFR)和疗效评分。结果3组治疗后JOA评分均较治疗前显著提高,且其中针药联合组JOA评分升高最为显著,差异存在统计学意义(P<0.05);治疗后,3组VAS评分均较治疗前明显降低,LFR较治疗前明显升高,且针药联合组较其他2组VAS评分更低,LFR更高,差异均存在统计学意义(P<0.05);治疗后,针药联合组总有效率为95.0%,在3组中疗效更优,差异存在统计学意义(P<0.05)。结论内热针与复元活血汤的联合使用优势互补,有效提高了腰椎间盘突出症的疗效,减轻腰部疼痛,改善腰部功能和腰屈曲程度,值得临床中推广使用。

Objective To investigate the clinical effect of internal heat needle combined with Fuyuan Huoxue decoction in patients with lumbar disc herniation.Methods A prospective collection of 120 patients with lumbar disc herniation from September 2017 to September2020 were randomly divided into acupuncture and drug combination group(treated with internal heat needle combined with Fuyuan Huoxue decoction),internal heat acupuncture group(treated with internal heat needle alone)and traditional Chinese medicine group(treated with Fuyuan Huoxue decoction alone),with 40 cases in each.After continuous treating 2~3 courses,the lumbar spine score(JOA),visual analogue score(VAS),lumbar flexion range(LFR)and curative effect scores were investigated 7 days and 3 months after the end of the course.Results The JOA scores of the three groups were significantly higher than those before treatment,and the JOA score of the combined group was the most significant,and there had statistically significant difference(P<0.05).After treatment,VAS scores of the three groups were significantly lower than before treatment,LFR significantly higher than before treatment,and the combined group had a lower VAS score and higher LFR levels than the other two groups,and all the differences were statistically significant(P<0.05).The total effective rate of the combined group was 95.0%,P<0.05 in the three groups.Conclusion The complementary combined use of internal heat needle and Fuyuan Huoxue decoction can effectively improve the efficacy of lumbar disc herniation,relief lumbar pain,and improve waist function and flexion,which is worth popularizing in clinic.
