胎盘多肽注射液联合负压封闭引流技术治疗儿童骨髓炎的临床研究

Clinical trail of placental polypeptide injection combined with negative pressure closed drainage technique in the treatment of pediatric osteomyelitis

目的 观察胎盘多肽注射液联合负压封闭引流技术(VSD)治疗儿童骨髓炎的临床疗效及安全性。方法 将54例患儿随机分为对照组27例和试验组27例。2组患儿均给予VSD清创处置+抗感染治疗;试验组在此基础上,给予胎盘多肽8 mL,qd,静脉滴注。2组患儿均连续治疗2周。比较2组患儿的肉芽组织生长时间、感染控制时间和总住院时间,以及药物不良反应的发生情况。结果 试验过程中脱落1例。试验组和对照组的总有效率分别为84.62%(22例/26例)和66.67%(18例/27例),差异有统计学意义(P<0.05)。治疗后,试验组和对照组的肉芽生长时间分别为(8.81±1.47)和(12.19±1.30)d,感染控制时间分别为(13.69±0.88)和(16.33±1.14)d,总住院时间分别为(36.81±2.43)和(42.67±3.37)d,差异均有统计学意义(均P<0.05)。试验组发生的药物不良反应有局部疼痛和荨麻疹,对照组发生的药物不良反应有局部疼痛和渗出液增多。试验组和对照组的总药物不良反应发生率分别为11.53%和7.41%,差异无统计学意义(P>0.05)。结论 胎盘多肽注射液联合VSD可抑制炎症反应,降低术后感染控制时间,临床疗效和安全性均较好。

Objective To observe the clinical efficacy and safety of placental peptide injection combined with vacuum sealing drainage(VSD) in the treatment of pediatric osteomyelitis. Methods Fifty-four children were randomly divided into control group(n=27 cases) and treatment group(n=27 cases). Two groups were given VSD debridement plus anti-infection therapy.On this basis, treatment group was given placental polypeptide 8 mL,qd,intravenous infusion. Two groups were treated for 2 weeks.The granulation growth time, infection control time, hospital stay and adverse drug reactions were compared betweentwo groups.Results One case fell off during the trail. The total effective rates of treatment and control groups were 84.62%(22 cases/26 cases) and 66.67%(18 cases/27 cases) with significant difference(P<0.05). After treatment, the granulation growth time of treatment and control groups was(8.81±1.47) and(12.19±1.30) days, the infection control time was(13.69±0.88) and(16.33±1.14) days, the hospital stay was(36.81±2.43) and(42.67±3.37) days, the differences were statistically significant(all P<0.05). The incidences of total adverse drug reactions in the treatment and control groups were 11. 53% and 7. 41%,and the difference was not statistically significant( P > 0. 05). Conclusion Placental polypeptide injection combined with VSD can inhibit inflammatory reaction and reduce postoperative infection control time,with good clinical efficacy and safety.
