KTH整合式干预在达格列净联合二甲双胍治疗肥胖/超重2型糖尿病中的效果

Intervention effects of dapagliflozin combined with metformin during KTH integrated nursing in treatment of obesity/overweight type 2 diabetes mellitus

目的分析达格列净联合二甲双胍治疗肥胖/超重2型糖尿病中KTH整合式护理干预效果分析。方法选取2020年6月到2021年3月收治的肥胖/超重2型糖尿病患者100例作为观察对象,根据病房号将其分为2组,对照组50例患者接受服达格列净+二甲双胍治疗,在对照组基础上,观察组患者接受KTH整合式护理干预,观察2组血脂、血糖及治疗前后脂肪因子水平。结果干预后,观察组Fins、FPG、HbA1c指标均优于对照组,差异有统计学意义(P<0.05);干预后2组血糖指标均下降,与干预前比较,差异有统计学意义(P<0.05)。干预后2组TG、TC均明显下降,与干预前对比,差异有统计学意义(P<0.05),观察组干预后LDL-C下降,HDL-C升高,与干预前对比,差异有统计学意义(P<0.05);干预前2组各血脂指标对比,差异无统计学意义(P<0.05);干预后,观察组HDL-C高于对照组,观察组LDL-C、TG、TC均对于对照组,差异有统计学意义(P<0.05)。干预后,2组患者CRP、内脂素均下降,脂联素水平均升高,与干预前对比差异有统计学意义(P<0.05);干预后观察组体重、BMI下降,与干预前对比,差异有统计学意义(P<0.05),干预后,观察组脂联素高于对照组,观察组CRP、体重、BMI与内脂素低于对照组,差异有统计学意义(P<0.5)。观察组患者接受KTH整合式护理,其不良反应发生率为6.00%,较对照组20.00%明显下降,差异有统计学意义(P<0.05)。结论肥胖/超重2型糖尿病患者达格列净联合二甲双胍治疗同时,联合KTH整合式护理干预,可改善患者血糖、血脂、脂肪因子水平,降低机体炎性反应,减少药物不良反应,值得推广。

Objective To investigate the clinical effects of dapagliflozin combined with metformin during KTH integrated nursing in treatment of obesity/overweight type 2 diabetes mellitus(T2DM).Methods One hundred patients with obesity/overweight T2DM who were treated in our hospital from June 2020 to March 2021 were enrolled in the study,who were divided into two groups according to the ward number,with 50 patients in each group.The patients in control group were treated by dapagliptin+metformin,however,those in observation group,on the basis of control group,were treated by KTH integrated nursing intervention.The levels of blood lipids,blood glucose and fat factors before and after treatment were observed and compared between the two groups.Results After the intervention,the Fins,FPG,HbA1c indexes in observation group were superior to those in control group(P<0.05),and the blood glucose indexes and the levels TG and TC in both groups were significantly decreased(P<0.05).After intervention,the LDL-C levels in observation group were decreased,but,the HDL-C levels were significantly increased(P<0.05).After intervention the blood lipid indexes,HDL-C,LDL-C,TG and TC in observation group were superior to those in control group(P<0.05).After the intervention,CRP,visfatin and adiponectin levels in both groups were decreased,and the adiponectin levels were significantly increased(P<0.05).After intervention,the body weight and BMI in observation group were significantly decreased(P<0.05).And the adiponectin levels in observation group were higher than those in control group,however,the levels of CRP,body weight,BMI and visfatin in observation group were significantly lower than those in control group(P<0.05).Moreover the incidence of adverse reactions in observation group was 6.00%,which was significantly lower than that(20.00%)in control group(P<0.05).Conclusion The dapagliflozin combined with metformin and KTH integrated nursing can improve the levels of blood glucose,blood lipid,fat factor,relieve the body inflammatory reaction,reduce the incidence of adverse drug reactions of patients with obesity/overweight T2DM,therefore,it is worthy of promotion in clinical practice.
