旋覆代赭汤合半夏厚朴汤治疗胃食管反流病合并胃肠道功能障碍临床研究

Clinical Study on Inula and Hematite Decoction Combined with Pinellia and Officinal Magnolia Bark Decoction in the Treatment of Gastroesophageal Reflux Disease Complicated with Gastrointestinal Dysfunction

目的:观察旋覆代赭汤合半夏厚朴汤治疗胃食管反流病(gastroesophageal reflux disease,GERD)合并胃肠道功能障碍的临床疗效。方法:将120例GERD合并胃肠道功能障碍患者按照随机数字表法分为观察组和对照组,每组各60例。对照组给予奥美拉唑肠溶片治疗,观察组在对照组治疗的基础上联合旋覆代赭汤合半夏厚朴汤。比较两组患者的临床疗效、不良反应发生率及治疗前后中医证候积分、胃肠激素[生长抑素(somatostatin,SS)、胃动素(motilin,MTL)、血管活性肠肽水平(vasoactive intestinal peptide,VIP)]水平变化情况。结果:观察组有效率为96.67%,对照组有效率为86.67%,两组有效率比较,差异具有统计学意义(P<0.05)。两组患者治疗后各项中医证候积分低于本组治疗前,且治疗后观察组低于对照组,差异具有统计学意义(P<0.05)。两组患者治疗后MTL、SP高于本组治疗前,SS、VIP低于本组治疗前,组间比较,差异均有统计学意义(P<0.05)。观察组不良反应发生率为5.00%,低于对照组的18.33%,差异具有统计学意义(P<0.05)。结论:旋覆代赭汤合半夏厚朴汤治疗GERD合并胃肠道功能障碍,可改善患者临床症状,提高胃肠道动力,降低不良反应发生率。

Objective:To observe the clinical efficacy of Inula and Hematite Decoction combined with Pinellia and Officinal Magno-lia Bark Decoction on gastroesophageal reflux disease(GERD)complicated with gastrointestinal dysfunction.Methods:A total of 120 patients with GERD complicated with gastrointestinal dysfunction were randomly divided into the observation group and the control group,with 60 cases in each group.The control group was treated with Omeprazole Enteric Coated Tablets,while the obser-vation group was additionally given Inula and Hematite Decoction combined with Pinellia and Officinal Magnolia Bark Decoction on the basis of the treatment of the control group.The clinical efficacy,the incidence of adverse reactions and the changes of TCM syndrome score,gastrointestinal hormones[somatostatin(SS),motilin(MTL)and vasoactive intestinal peptide(VIP)]before and after treatment were compared between the two groups.Results:The effective rate of the observation group was 96.67%and that of the control group was 86.67%.The difference between the two groups was statistically significant(P<0.05).The TCM syndrome scores of the two groups after treatment were lower than those of the group before treatment,and the TCM syndrome scores of the observation group after treatment were lower than those of the control group,and all the differences were statistically significant(P<0.05).After treatment,MTL and SP of the two groups were higher than those before treatment,while SS and VIP were lower than those before treatment,and all the differences between the two groups were statistically significant(P<0.05).The incidence of adverse reactions in the observation group was 5.00%,which was lower than 18.33%in the control group,and the difference was statistically significant(P<0.05).Conclusion:Inula and Hematite Decoction combined with Pinellia and Offi-cinal Magnolia Bark Decoction in the treatment of GERD complicated with gastrointestinal dysfunction can better the clinical symp-toms,improve gastrointestinal function and reduce the incidence of adverse reactions.
