不同美学修复术治疗乳前牙龋病的临床研究

Clinical study of different aesthetic restorations in the treatment of caries of deciduous anterior teeth

目的 研究应用三种不同美学修复术治疗乳前牙龋坏的临床效果,为儿童乳前牙龋坏临床治疗提供有参考价值的资料。方法选取2020年6月至2021年6月于本院就诊的上颌乳切牙患龋需要治疗的儿童189名,共300颗龋坏上颌乳切牙,随机分为3组,100颗应用乳前牙全瓷贴面进行修复(A组);100颗应用透明树脂冠进行修复(B组);100颗应用乳前牙预成全瓷冠进行修复(C组)。在治疗后3月、6月及12个月分别对修复体脱落率、牙龈出血状况、继发龋发生率、修复体破损率、患者满意程度进行临床效果评价。结果 术后3个月、6个月三种修复方式治疗成功率、修复体脱落率、牙龈出血率、继发龋发生率、修复体破损率,差异无统计学意义(P>0.05);术后12个月三种修复手术方式治疗成功率、修复体脱落率、牙龈出血率差异有统计学意义(P<0.05)。全瓷贴面满意度评分显著高于透明树脂冠、预成全瓷冠,差异有统计学意义(P<0.05)。结论 全瓷贴面修复乳前牙操作简单,修复成功率高,值得在临床上推广。

Objective To study the clinical effect of three different aesthetic restorations in the treatment of caries of deciduous anterior teeth, and to provide valuable reference data for the clinical treatment of caries of deciduous anterior teeth in children. Methods A total of 300carious maxillary deciduous incisors were selected from 189 children who were treated in our hospital from June 2020 to June 2021 with caries in maxillary deciduous incisors and were randomly divided into 3 groups. Anterior teeth were restored with all-ceramic veneers(group A), 100 were re-stored with transparent resin crowns(group B), 100 were restored with prefabricated all-ceramic crowns from primary anterior teeth(group C). At 3, 6,and 12 months after treatment, the clinical effect was evaluated on the restoration rate, gingival bleeding, secondary caries incidence, restoration dam-age rate, and patient satisfaction. Results There was no significant difference in the treatment success rate, prosthesis shedding rate, gingival bleeding rate, secondary caries incidence rate, and prosthetic damage rate at 3 months and 6 months after operation( P>0.05). There were significant differences in the treatment success rate, prosthesis fall-off rate, and gingival bleeding rate among the three types of revision surgery at 12 months af-ter surgery(P<0.05). The satisfaction score of all-ceramic veneers was significantly higher than that of transparent resin crowns and prefabricated allceramic crowns, and the difference was statistically significant(P<0.05). Conclusion All-ceramic veneer is easy to operate and has a high restoration success rate in the restoration of deciduous anterior teeth, which is worthy of clinical promotion.
