外周血CD_(8)^(+)CD_(28)^(-)T细胞Foxp3 mRNA表达与老年肺部感染的关系

Relationship between Foxp3 mRNA expression in peripheral blood and CD_(8)^(+)CD_(28)^(-)T cells and lung infection in elder patients

目的分析外周血CD_(8)^(+)CD_(28)^(-)T细胞叉头框蛋白p3(Foxp3)mRNA表达与老年肺部感染的关系。方法选择2019年12月-2021年12月乌鲁木齐市友谊医院收治的老年慢性支气管炎患者200例,根据患者是否合并肺部感染分为感染组(n=135)和未感染组(n=65),比较感染组和未感染组患者及不同肺部感染程度患者外周血CD_(8)^(+)CD_(28)^(-)T细胞水平和Foxp3 mRNA表达水平,检测感染组和未感染组患者血清炎性因子[白细胞介素-6(IL-6)、白细胞介素-10(IL-10)、干扰素-γ(IFN-γ)和转化生长因子-β1(TGF-β1)]水平,分析老年肺部感染患者外周血CD_(8)^(+)CD_(28)^(-)T细胞水平和Foxp3 mRNA表达水平与血清炎性因子水平、临床肺部感染评分(CPIS)的关系。结果感染组患者外周血CD_(8)^(+)CD_(28)^(-)T细胞水平和Foxp3 mRNA表达水平均高于未感染组患者(P<0.05);不同肺部感染程度患者外周血CD_(8)^(+)CD_(28)^(-)T细胞水平和Foxp3 mRNA表达水平比较,差异具有统计学意义(P<0.05),中、重度感染组高于轻度感染组(P<0.05),重度感染组高于中度感染组(P<0.05);感染组患者血清IL-6、IFN-γ水平均高于未感染组患者(P<0.05),IL-10、TGF-β1水平均低于未感染组患者(P<0.05);Pearson相关检验结果显示,CD_(8)^(+)CD_(28)^(-)T细胞水平和Foxp3 mRNA表达水平与血清IL-6、IFN-γ水平、CPIS评分呈正相关(P<0.05),与IL-10、TGF-β1水平呈负相关(P<0.05)。结论老年肺部感染患者外周血CD_(8)^(+)CD_(28)^(-)T细胞Foxp3 mRNA表达显著上调,且与肺部感染程度存在密切关系。

OBJECTIVE To analyze the relationship between mRNA expression of fork-head box protein p3(Foxp3)and CD_(8)^(+)CD_(28)^(-)T cells in peripheral blood and lung infection in elder patients.METHODS Total of 200 elderly patients with chronic bronchitis admitted to Urumqi Friendship Hospital between Dec 2019 to Dec 2021 were recruited and divided into the infected group(n=135)and uninfected group(n=65)according to whether they were complicated with lung infection.The percentage of peripheral blood CD_(8)^(+)CD_(28)^(-)T cells and expression level of Foxp3 mRNA between the infected group and uninfected group and among patients with different degrees of lung infection were compared.The levels of serum inflammatory factors including interleukin-6(IL-6),interleukin-10(IL-10),interferon-gamma(IFN-γ)and transforming growth factor-β1(TGF-β1)were detected between the infected group and uninfected group.The correlations between levels of peripheral blood CD_(8)^(+)CD_(28)^(-)T cells and Foxp3 mRNA expression and levels of serum inflammatory factors and clinical pulmonary infection score(CPIS)among the elderly patients with lung infection were analyzed.RESULTS The level of peripheral blood CD_(8)^(+)CD_(28)^(-)T cells and expression level of Foxp3 mRNA in the infected group were significantly higher than those in the uninfected group(P<0.05).There were significant differences in the levels of peripheral blood CD_(8)^(+)CD_(28)^(-)T cells and Foxp3 mRNA expression among patients with different lung infection grades(P<0.05),and the two indicators in the moderate infection group and severe infection group were significantly higher than those in the mild infection group(P<0.05),and those in the severe infection group were significantly higher than those in the moderate infection group(P<0.05).The levels of serum IL-6 and IFN-γin the infected group were significantly higher than those in the uninfected group(P<0.05)while the levels of IL-10 and TGF-β1 were significantly lower than those in the uninfected group(P<0.05).Pearson correlation test showed that CD_(8)^(+)CD_(28)^(-)T cell level and Foxp3 mRNA expression level were positively correlated with levels of serum IL-6 and IFN-γand CPIS score(P<0.05),and were negatively correlated with levels of IL-10 and TGF-β1(P<0.05).CONCLUSION The expression of Foxp3 mRNA of peripheral blood CD_(8)^(+)CD_(28)^(-)T cells in elderly patients with lung infection is significantly up-regulated,and it is closely related to the degree of pulmonary infection.
