Effect of PDCA cycle on hand hygiene of nursing staff in a county hospital of Tibet

目的 探讨运用计划-实施-检查-处理,PDCA循环管理提高西藏左贡县医院护理人员手卫生依从性及正确率的效果。方法 以2020年1月-6月在西藏左贡县人民医院工作的护理人员为调研对象,以提高手卫生依从性和正确率为目的开展PDCA循环管理,完善管理制度、建立管理体系、加强培训教育,对比PDCA循环前后护理人员手卫生依从性及正确率的变化。结果 通过本次PDCA循环管理,在手卫生5大指征较实施前均有提高,尤其是在接触患者前手卫生依从性提高幅度最大,从25.00%提高到62.50%,无菌操作前依从性提高到100.00%,手卫生管理效果明显;且PDCA循环后护理人员手卫生正确率高于PDCA循环前,由50.00%提高至78.13%,差异有统计学意义(P<0.05)。结论 在西藏欠发达地区县医院开展PDCA循环管理,通过完善制度、建立管理体系、培训教育等措施,能够提高护理人员手卫生依从性,对于防控医院感染,保障医疗安全、维护医疗质量具有重要意义。

OBJECTIVE To explore the effect of plan-do-check-action) cycle management on improving the hand hygiene compliance and accuracy of nursing staff in Zuogong County Hospital of Tibet. METHODS From Jan 2020 to Jun 2020, the nursing staff were recruited from Zuogong County Hospital of Tibet. PDCA cycle management was carried out so as to improve the hang hygiene compliance and accuracy, perfect the management system, establish the management system and strengthen the education. The compliance rate and accurate rate of hand hygiene of the nursing staff were compared before and after the PDCA was carried out. RESULTS The five indications for hand hygiene were improved after the PDCA cycle was carried out, the hand hygiene compliance was improved most significantly before contacting patients, increasing from 25.00% to 62.50%, the hand hygiene compliance before aseptic operation was increased to 100.00%, and the hand hygiene management was improved remarkably. The accurate rate of hand hygiene was higher after the PDCA cycle than before the PDCA cycle, increasing from 50.00% to 78.13%, and there was significant difference(P<0.05). CONCLUSION The PDCA cycle management is carried out in the less developed area of Tibet so as to perfect the system, establish the management systems and strengthen the training, which may raise the hand hygiene compliance of the nursing staff and have great significance in preventing nosocomial infection, ensuring medical safety and maintaining the medical quality.
