自制一次性密闭式瘤体取出保护套在腹腔镜下子宫肌瘤分碎术中的应用

Application of a disposable protective sleeve for closed tumor removal in laparoscopic hysteromyoma dissection

目的探讨自制一次性密闭式瘤体取出保护套在腹腔镜下子宫肌瘤分碎术中应用的有效性及安全性。方法选取2019年12月1日至2022年3月31日于中国科学技术大学附属第一医院(安徽省立医院)采用自制一次性密闭式瘤体取出保护套实施腹腔镜下子宫肌瘤分碎术的63例患者,分析手术时间、标本袋完整情况、术中出血及术后恢复情况,评估使用自制一次性密闭式瘤体取出保护套的有效性及安全性。结果63例患者的年龄20~48(36.7±6.2)岁,肌瘤数目1~6(1.41±0.99)个,肌瘤长径范围5~12(7.45±1.95)cm;所有患者均顺利实施腹腔镜下子宫肌瘤挖除术及自制一次性密闭式瘤体取出保护套内肌瘤旋切取出术,无瘤体取出套破裂情况;手术时间40~180(89.14±27.45)min,术中出血量范围10~100(34.29±18.46)mL;术后恢复情况良好,无不良事件发生,住院时间5~12(7.60±1.46)d。结论自制一次性密闭式瘤体取出保护套在腹腔镜下子宫肌瘤分碎术中操作便捷、自制过程简单、成本较低,在多发或单发直径10 cm以内的子宫肌瘤分碎术中手术效果满意。

Objective To investigate the efficacy and safety of the self-made protective sleeve for closed tumor removal in laparoscopic hysteromyoma dissection.Methods We selected 63 patients who were usd self-made protective sleeves for closed tumor removal in laparoscopic hysteromyoma dissection from December 1,2019 to March 31,2022 at the First Affiliated Hospital of the University of Science and Technology of China(Anhui Provincial Hospital),We also analyzed the operation time,Specimen bag complete condition,intraoperative bleeding and postoperative recovery,and evaluated the efficacy and safety of a self-made disposable closed tumor removal sheath.Results The age of 63 patients ranged from^(2)0 to 48(36.7±6.2)years old,the number of fibroids ranged from 1 to 6(1.41±0.99),and the length and diameter of fibroids ranged from 5 to 12(7.45±1.95)cm,all patients were successfully implemented Laparoscopic myomectomy and self-made one-time closed tumor removal protective sleeve myomectomy,no rupture of the tumor removal sleeve,operation time ranged from 40 to 180(89.14±27.45)min.The median bleeding range was 10-100(34.29±18.46)mL,the postoperative recovery was good,no adverse events occurred,and the average hospital stay was 5-12(7.60±1.46)days.Conclusions The self-made disposable closed tumor removal protective sleeve is convenient,simple and low-cost in laparoscopic uterine fibroids fragmentation,and the surgical effect is satisfactory in multiple or single uterine fibroids with in 10 cm.
