健脾益气化瘀汤联合四联疗法治疗Hp感染慢性萎缩性胃炎的效果

Effect of Jianpi Yiqi Huayu Decoction Combined with Quadruple Therapy on Chronic Atrophic Gastritis Infected by Hp

目的:探讨健脾益气化瘀汤联合四联疗法治疗幽门螺杆菌感染慢性萎缩性胃炎的效果。方法:选择2020年10月-2021年12月福建医科大学附属闽东医院收治的120例幽门螺杆菌感染致慢性萎缩性胃炎患者,根据随机数字表法分为对照组和观察组,各60例,其中对照组采用四联疗法(雷贝拉唑钠肠溶片、阿莫西林胶囊、呋喃唑酮片、枸橼酸铋钾片)治疗,观察组在对照组基础上采用健脾益气化瘀汤治疗。比较两组临床效果、临床症状积分、胃镜下表现积分和病理结果积分,以及相关血清炎症因子水平。结果:治疗后观察组的总有效率为91.67%,明显高于对照组的78.33%,差异有统计学意义(P<0.05)。治疗后两组的疲乏无力、胃痛、反酸、大便稀溏等临床症状积分、病理结果及胃镜下表现积分均显著降低,且观察组较对照组降低更显著(P<0.05)。治疗后两组的血清肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)、白介素-10(IL-10)水平均降低,且观察组较对照组降低更显著,差异有统计学意义(P<0.05)。两组的不良反应发生率比较,差异无统计学意义(P>0.05)。结论:健脾益气化瘀汤联合四联疗法治疗幽门螺杆菌感染慢性萎缩性胃炎的效果显著,可减轻炎症反应,从而改善临床症状,促进预后。

Objective:To explore the effect of Jianpi Yiqi Huayu Decoction combined with quadruple therapy on chronic atrophic gastritis infected by Helicobacter pylori.Method:A total of 120 patients with chronic atrophic gastritis caused by Helicobacter pylori infection were selected from the Mindong Hospital Affiliated to Fujian Medical University from October 2020 to December 2021.According to the random number table method,they were divided into control group and observation group,with 60 cases in each group.The control group was treated with quadruple therapy(Rabeprazole Sodium Enteric Coated Tablets,Amoxicillin Capsules,Furazolidone Tablets,Bismuth Potassium Citrate Tablets),while the observation group was treated with the Jianpi Yiqi Huayu Decoction on the basis of the control group.The clinical curative effect,clinical symptom scores,scores of gastroscopy and pathological results,and the levels of related serum inflammatory factors were compared between the two groups.Result:After treatment,the total effective rate of the observation group was 91.67%,significantly higher than 78.33%of the control group,and the difference was statistically significant(P<0.05).After treatment,the scores of clinical symptoms,such as fatigue,stomachache,acid reflux,loose stool,pathological results and gastroscopy scores of the two groups were significantly reduced,and the observation group decreased more significantly than the control group(P<0.05).After treatment,the levels of serum tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)and interleukin-10(IL-10)in the two groups decreased,the observation group decreased more significantly than the control group,and the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:The Jianpi Yiqi Huayu Decoction combined with quadruple therapy is effective in treating chronic atrophic gastritis infected by Helicobacter pylori,which can reduce inflammatory reaction,improve clinical symptoms and promote prognosis.
