腹膜外腹腔镜疝气修补术治疗腹股沟疝的效果

Effect of Extraperitoneal Laparoscopic Hernia Repair in the Treatment of Inguinal Hernia

目的:探讨腹膜外腹腔镜疝气修补术治疗腹股沟疝(IH)的效果。方法:选取2020年3月-2021年4月绵阳市中心医院收治的共计76例IH患者,按照随机摸球法分成A组(38例)与B组(38例),B组实施开放性无张力疝气修补术,A组实施腹膜外腹腔镜疝气修补术,比较两组临床指标、应激反应指标及并发症发生率。结果:与B组相比,A组术中出血量明显更少(P<0.05),手术时间、住院时间、切口长度明显更短(P<0.05),切口疼痛评分明显更低(P<0.05);与术前相比,两组术后1 d皮质醇(Cor)、去甲肾上腺素(NE)、醛固酮(ALD)水平均明显提高(P<0.05),但与B组相比,A组水平明显更低(P<0.05);与B组相比,A组并发症发生率明显更低(P<0.05)。结论:腹膜外腹腔镜疝气修补术治疗IH患者,可改善临床指标,减少并发症的发生,且对应激反应指标的影响较小。

Objective:To explore the effect of extraperitoneal laparoscopic hernia repair on inguinal hernia(IH).Method:A total of 76 patients with inguinal hernia who were admitted in Mianyang Central Hospital from March 2020 to April 2021 were selected,and they were divided into group A(38 cases)and group B(38 cases)according to the random touch ball method.Group B was implemented open-ended tension-free herniorrhaphy,and group A was performed extraperitoneal laparoscopic hernia repair.The clinical indicators,stress response indicators,and complication rates of the two groups were compared.Result:Compared with group B,intraoperative blood loss in group A was significantly less(P<0.05),the operation time,hospital stay,and incision length were significantly shorter(P<0.05),and the incision pain score was significantly lower(P<0.05).Compared with before surgery,the levels of cortisol(Cor),norepinephrine(NE),and aldosterone(ALD)in the two groups were significantly increased at 1 day after surgery(P<0.05),but compared with group B,the levels in group A were significantly lower(P<0.05).The complication rate of group A was significantly lower than that of group B(P<0.05).Conclusion:Extraperitoneal laparoscopic hernia repair in the treatment of IH patients can improve clinical indicators,reduce the occurrence of complications,and has a small impact on stress response indicators.
