套叠多支架桥接技术治疗大脑中动脉远端巨大夹层动脉瘤一例

Overlapped multi-stent bridging technique in the treatment of huge dissecting aneurysm of the distal middle cerebral artery

大脑中动脉远端巨大夹层动脉瘤的血管内治疗中,如何保持载瘤动脉通畅,依然是一大挑战。该文报道1例大脑中动脉远端巨大夹层动脉瘤,使用套叠3枚LEO支架进行血管重建,术后即刻造影示载瘤动脉通畅,动脉瘤体显影减少。术后9个月造影随访显示动脉瘤完全不显影。对于大脑中动脉远端长径巨大夹层动脉瘤,采用套叠LEO支架可发挥较好的血流导向和血管重建作用,具有更好的经济效益。

Endovascular treatment of huge dissecting aneurysms at the distal end of the middle cerebral artery remains a major challenge,especially how to maintain the patency of the parent artery.This article reported a case of this kind.Three overlapping LEO stents were used for vascular reconstruction.Immediate angiographic results and 9-month follow-up showed that the parent artery was unobstructed and the volume of aneurysm was reduced.Angiographic follow-up at 9 months after operation showed that the aneurysm was totally obliterated.For huge dissecting aneurysms with long diameter,the use of overlapping LEO stents can exert better blood flow guidance and vascular remodeling effects,and have better economic benefits.
