解毒痤疮丸联合异维A酸红霉素凝胶对寻常痤疮中医证候积分及血清SIL-2R、IL-17、Caspase-1水平的影响 被引量:8

摘要 目的:探究解毒痤疮丸联合异维A酸红霉素凝胶对寻常痤疮患者中医证候积分及血清可溶性白细胞介素-2受体(SIL-2R)、白细胞介素-17(IL-17)、半胱氨酸蛋白酶-1(Caspase-1)水平的影响。方法:选取2019年1月-2020年6月笔者医院126例寻常痤疮患者,随机分为对照组与观察组,各63例。对照组予以异维A酸红霉素凝胶治疗,观察组予以解毒痤疮丸联合异维A酸红霉素凝胶治疗,均治疗8周。对比两组临床疗效、不良反应发生率及中医证候积分、皮损评分、血清SIL-2R、IL-17、Caspase-1水平、外周血T淋巴细胞亚群、经皮水分丢失值、角质层含水量、pH值。结果:治疗8周后观察组临床治疗总有效率(93.65%vs 80.95%)高于对照组(P<0.05);治疗4周、8周后,观察组中医证候积分、皮损评分、血清SIL-2R、IL-17、Caspase-1水平均低于对照组(P<0.05);治疗4周、8周后,观察组CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)、角质层含水量高于对照组,CD8^(+)、经皮水分丢失值、pH值低于对照组(P<0.05);两组不良反应发生率相比,差异无统计学意义(P>0.05)。结论:解毒痤疮丸、异维A酸红霉素凝胶治疗寻常痤疮,可显著改善患者症状,调节机体免疫炎性反应,促进皮肤屏障功能恢复,疗效显著,值得临床推广与应用。 Objective To explore the effects of Acne Detox Pills combined with erythromycin isotretinoin gel on TCM syndrome scores and serum soluble interleukin-2 receptor(SIL-2R)and interleukin-17(IL-17)in patients with acne vulgaris,the influence of Caspase-1 levels.Methods A total of 126 patients with acne vulgaris in author hospital from January 2019 to June 2020 were selected and randomly divided into a control group and an observation group,with 63 cases in each group.The control group was treated with erythromycin isotretinoin gel,and the observation group was treated with Acne Detox Pills combined with erythromycin isotretinoin gel for 8 weeks.Statistically compare the clinical efficacy,incidence of adverse reactions,TCM syndrome scores,skin lesion scores,serum SIL-2R,IL-17,Caspase-1 levels,and peripheral blood T lymphocytes.Subgroups,transdermal water loss value,stratum corneum water content,pH value.Results After 8 weeks of treatment,the total effective rate of clinical treatment in the observation group(93.65%vs 80.95%)was higher than that of the control group(P<0.05),after 4 weeks and 8 weeks of treatment,the observation group’s TCM syndrome score,skin lesion score,and serum SIL-2R,IL-17,and Caspase-1 levels were lower than those of the control group(P<0.05),after 4 weeks and 8 weeks of treatment,the CD3^(+),CD4^(+),CD4^(+)/CD8^(+)and cuticle water content of the observation group were higher than those of the control group,and CD8^(+)The value of transdermal water loss and pH value were lower than those of the control group(P<0.05),there was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Acne Detox Pills and erythromycin isotretinoin gel in the treatment of acne vulgaris can significantly improve the symptoms of patients,regulate the body’s immune inflammatory response,and promote the recovery of skin barrier function.The curative effect is significant,and it is worthy of clinical promotion and application.
