4E三维动作捕捉系统配合rTMS对脑卒中患者上肢运动功能的影响

摘要 目的观察4E三维动作捕捉系统配合rTMS改善脑卒中患者的上肢运动功能及手功能的效果。方法选取脑卒中后手功能障碍患者60例,按随机数字法分为3组,即rTMS组、4E组和观察组,每组20例,所有患者均给予正常的药物治疗与康复训练,rTMS组增加rTMS进行上肢功能训练,4E组增加4E三维动作捕捉系统治疗,观察组患者先接受rTMS治疗,结束后立即进行4E三维动作捕捉系统训练,各组治疗1次/d,每周5d,一个疗程为4周,持续治疗一个疗程。治疗前后三组患者均采用简化FuglMeyer运动功能评定量表上肢部分(FMA-UE)、Carroll上肢功能测试(UEFT)及改良Barthel指数(MBI)进行评定。结果治疗前,三组患者FMA-UE、UEFT、MBI的评分对比,差异无统计学意义(P>0.05)。三组组内治疗前后比较,FMA-UE、UEFT、MBI评分均有所改善(P<0.05);与rTMS组治疗后比较,4E组及观察组上述指标均显著改善(P<0.05),观察组与4E组比较,观察组治疗后FMA-UE为(51.45±1.87)分、UEFT为(34.75±2.98)分、MBI评分为(67.55±3.23)分,均优于4E组(P<0.05)。结论4E三维动作捕捉系统配合rTMS可以显著改善脑卒中患者的上肢运动功能、手功能与ADL能力。 Objective To observe the effect of 4 E three-dimensional motion capture system combined with rTMS on upper limb motor function and hand function in patients with stroke.Methods 60 patients with hand dysfunction after stroke were randomly divided into three groups:rTMS group,4 e group and observation group.All patients were still undergoing normal drug and rehabilitation training,with 20 patients in each group.rTMS group was added with rTMS for upper limb function training,4 e group was added with 4 E three-dimensional motion capture system,and the patients in the observation group were treated with rTMS first,The 4 E 3 D motion capture system was trained immediately after the training.The patients were assessed with the simplified fuglmeyer motor function assessment scale upper limb part(FMA-UE),Carroll upper limb function test(UEFT)and modified Barthel Index(MBI),and then treated once a day,5 days a week for a course of treatment for 4 weeks.After treatment,the patients were evaluated with these three scales again.Results Before treatment,the scores of FMA-UE,UEFT and MBI of the three groups were compared,and the difference was not statistically significant(P>0.05).After treatment with these three groups,each group was compared before and after treatment.The scores of FMA-UE,UEFT and MBI of these three groups were improved(P<0.05),and then compared between groups.After treatment,the above indexes of 4 E group and rTMS group were significantly improved(P<0.05).Comparing the scores of the observation group and 4 E group,we found that the FMA-UE(51.45±1.87),UEFT(34.75±2.98)and MBI score(67.55±3.23)of the observation group were excellent after treatment(P<0.05).Conclusion 4 E 3 D motion capture system combined with rTMS can significantly improve the upper limb motor function,hand function and ADL ability of stroke patients.
