摘要
目的分析围绝经期女性腰椎骨质疏松症的椎体脂肪分数(fatfraction,FF)与骨密度(bone mineral density,BMD)的相关性,探讨MR脂肪定量最小二乘估计和不对称回波迭代分解水和脂肪成像(iterativede composition of water and fat with echo asymmetryand least squares estimation quantification sequence,IDEAL-IQ)对围绝经期女性在腰椎骨质疏松症临床诊断中的研究价值。方法2019年12月—2021年6月收集围绝经期女性患者60例,同时行双能X线骨密度仪(dual X-ray absorptiometry,(DXA)和脂肪定量最小二乘估计和不对称回波迭代分解水和脂肪成像(IDEAL-IQ)序列测量腰椎椎体(L^(1)-L_(4))骨密度值(T值)和FF值。根据T值将研究分为骨量正常组(T≥-1.0)、骨量减少组(-2.5<T<-1.0)和骨质疏松组(T≤-2.5)。用单因素方差分析(analysis of variance,ANOVA)比较三组之间FF差异,每两组之间采用Bonferroni多重比较,以P<0.05为差异有统计学意义。采用Pearson相关分析评估BMD、FF及年龄之间相关性。结果骨质疏松组的FF值显著高于骨量正常组和骨量减少组,差异有统计学意义(P<0.05)。L_(1-4)骨密度与FF、年龄呈负相关(r=-0.468,P=0.000)(r=-0.447,P=0.002);年龄和FF呈正相关(r=0.610,P=0.000)。结论通过IDEAL-IQ技术可以定量分析腰椎FF值有助于评价骨强度,对于椎骨质疏松早期诊断有重要作用,能够更好对患者进行评估分析,使患者对自己病情有更加全面的了解。
Objective To analyze the correlation between fatfraction(FF)and bone mineral density(BMD)in perimenopausal women with lumbar osteoporosis,and to explore the value of MR fat quantitative least square estimation and asymmetric echo iterative decomposition of water and fat imaging(IDEAL-IQ)in the clinical diagnosis of lumbar osteoporosis in perimenopausal women.Methods From December 2019 to June 2021,sixty perimenopausal female patients were collected and treated with dual X-ray absorptiometry,(DXA)and fat quantitative least squares estimation and iterative composition of water and fat with echo asymmetry and least squares estimation quantification sequence(IDEAL-IQ)was used to measure BMD values(T values)and FF values of the lumbar vertebral body(L_(1)-L_(4)).According to the t-value,the study was divided into normal bone mass group(T≥-1.0),decreased bone mass group(-2.5<T<-1.0)and osteoporosis group(T≤-2.5).Analysis of variance(ANOVA)was used to compare FF differences among the three groups,and Bonferroni multiple comparison was used between the two groups.P<0.05 was considered statistically significant.Pearson correlation analysis was used to evaluate the correlation between BMD,FF and age.Results FF value in osteoporosis group was significantly higher than that in normal bone mass group and reduced bone mass group,and the difference was statistically significant(P<0.05).L_(1-4) bone mineral density was negatively correlated with FF and age(r=-0.468,P=0.000)and(r=-0.447,P=0.002).Age was positively correlated with FF(r=0.610,P=0.000).Conclusion Quantitative analysis of lumbar FF va lues through IDEAL-IQ technology can help evaluate bone strength and play an important role in the early diagnosis of lumbar osteoporosis,so that patients can be better assessed and analyzed and have a more comprehensive understanding of their condition.
