静脉注射不同剂量免疫球蛋白对新生儿肺炎的疗效及其免疫抑制作用 被引量:1

摘要 目的探究静脉注射不同剂量免疫球蛋白对新生儿肺炎患儿的疗效及其免疫抑制作用。方法选取新密市妇幼保健院2019年12月至2021年12月接诊的82例新生儿肺炎患儿作为研究对象,采用随机抽样法作为分组基础,分为常规剂量组(常规剂量免疫球蛋白静脉注射)和大剂量组(大剂量免疫球蛋白静脉注射)各41例,对两组疗效、动脉血气指标、免疫功能、炎症因子进行分析比较。结果大剂量组治疗效果高于常规剂量组,差异有统计学意义(P<0.05),分别为97.56%、82.93%,用药前两组动脉血气指标比较,差异无统计学意义(P>0.05),用药后大剂量组氧分压高于常规剂量组,差异有统计学意义(P<0.05),二氧化碳分压水平低于常规剂量组,差异有统计学意义(P<0.05),用药前两组各项免疫功能指标水平比较,差异无统计学意义(P>0.05),用药后大剂量组免疫球蛋白A(IgA)、免疫球蛋白G(IgG)、免疫球蛋白M(IgM)水平均高于常规剂量组,差异有统计学意义(P<0.05),用药前两组炎症因子指标比较,差异无统计学意义(P>0.05),用药后大剂量组白细胞、白细胞介素-6、肿瘤坏死因子-α水平均低于常规剂量组,差异有统计学意义(P<0.05)。结论相比常规剂量,大剂量人免疫丙种球蛋白治疗新生儿肺炎疗效显著,可显著改善炎症因子、免疫功能,值得应用。 Objective To explore the efficacy and immunosuppressive effect of intravenous immunoglobulin in different doses on neonatal pneumonia.Methods A total of 82 children with neonatal pneumonia who were admitted to Xinmi maternal and child health hospital from December 2019 to December 2021 were selected as the study subjects.Random sampling method was used as the grouping basis,and they were divided into the routine dose group(routine dose immunoglobulin intravenous injection)and the high dose group(high dose immunoglobulin intravenous injection)with 41 cases each.The efficacy,arterial blood gas index,immune function and inflammatory factors of the two groups were analyzed and compared.Results The therapeutic effect of the high-dose group was higher than that of the routine dose group,and the difference was statistically significant(P<0.05),which was 97.56%and 82.93%respectively.There was no significant difference in the arterial blood gas indexes between the two groups before treatment(P>0.05).After treatment,the oxygen partial pressure of the high-dose group was higher than that of the routine dose group,and the difference was statistically significant(P<0.05).The carbon dioxide partial pressure level was lower than that of the routine dose group,and the difference was statistically significant(P<0.05).There was no significant difference in the levels of immune function indexes between the two groups before the treatment(P>0.05).After the treatment,the levels of immunoglobulin A(IgA),immunoglobulin G(IgG)and immunoglobulin M(IgM)in the high-dose group were higher than those in the routine dose group,and the difference was statistically significant(P<0.05).There was no significant difference in the inflammatory factor indexes between the two groups before the treatment(P>0.05),The levels of WBC,IL-6 and TNF-αin the high-dose group were lower than those in the routine dose group(P<0.05).Conclusion Compared with routine dose,high dose human immunoglobulin has a significant effect on neonatal pneumonia,which can significantly improve inflammatory factors and immune function,and is worthy of application.
