标本溶血对生化检验中心肌酶、血脂、电解质、肝功能指标的影响 被引量:7

Effect of Specimen Hemolysis on the Indexes of myocardial Enzyme,Blood Lipid,Electrolyte and liver Function in biochemical Test Center

摘要 目的分析使用溶血标本对生化检验中肝功能、电解质、血脂以及心肌酶指标检测的影响。方法选择2020年5月至2021年12月在河南省濮阳县人民医院开展生化检验的患者,共计82例。全部患者均采集血液样本,并将其均分成两管,即无溶血标本(未溶血组)和溶血标本(溶血组),并依据溶血程度将溶血组患者划分成轻中度溶血组与重度溶血组。对比不同溶血程度组与未溶血组血液样本肝功能、电解质、血脂及心肌酶指标水平。结果在总蛋白、天门冬氨酸氨基转移酶以及丙氨酸氨基转移酶水平方面,轻中度溶血组、重度溶血组血液样本中上述指标均高于未溶血组,重度溶血组血液样本中上述指标均高于轻中度溶血组,差异有统计学意义(P<0.05)。在钙离子、氯离子以及钾离子水平方面,轻中度溶血组、重度溶血组血液样本中上述指标均高于未溶血组,重度溶血组血液样本中上述指标均高于轻中度溶血组,差异有统计学意义(P<0.05)。在甘油三酯以及总胆固醇水平方面,轻中度溶血组、重度溶血组血液样本中上述指标均高于未溶血组,重度溶血组血液样本中上述指标均高于轻中度溶血组,差异有统计学意义(P<0.05)。肌钙蛋白I、前体N末端前脑利钠肽、肌酸激酶同工酶以及肌酸激酶水平方面,轻中度溶血组、重度溶血组血液样本中上述指标均高于未溶血组,重度溶血组血液样本中上述指标均高于轻中度溶血组,差异有统计学意义(P<0.05)。结论生化检验中肝功能、电解质、血脂以及心肌酶指标检测时使用溶血标本,会对检测结果造成显著影响,为保障检测结果的准确性,需规范使用血液标本进行检测。 Objective To analyze the influence of specimen hemolysis on the indexes of liver function,electrolyte,blood lipid and myocardial enzyme in biochemical test.Methods From May 2020 to December 2021,a total of 82 patients were selected from People’s Hospital of Puyang County,Henan Province.Blood samples were collected from all patients and divided into two tubes,no hemolysis sample(non-hemolysis group)and hemolysis sample(hemolysis group).According to the hemolysis degree,patients in hemolysis group were divided into mild to moderate hemolysis group and severe hemolysis group.The levels of liver function,electrolyte,blood lipid and myocardial enzyme in blood samples of different hemolysis groups and nonhemolysis groups were compared.Results The levels of total protein,aspartate aminotransferase and alanine aminotransferase in blood samples of mild to moderate hemolysis group and severe hemolysis group were higher than those of non-hemolysis group,the above-mentioned indexes in severe hemolysis group were higher than those in mild to moderate hemolysis group,the difference was statistically significant(P<0.05).The levels of calcium,chloride and potassium in the blood samples of the mild to moderate hemolytic group and the severe hemolytic group were higher than those of the non-hemolytic group,the abovementioned indexes in severe hemolysis group were higher than those in mild to moderate hemolysis group,the difference was statistically significant(P<0.05).In terms of triglyceride and total cholesterol levels,the above-mentioned indexes in the mild to moderate hemolytic group and the severe hemolytic group were higher than those in the non-hemolytic group,and the abovementioned indexes in the severe hemolytic group were higher than those in the mild to moderate hemolytic group,the difference was statistically significant(P<0.05).Levels of troponin I,N-terminal NPPB,creatine kinase isozymes and creatine kinase in mild to moderate hemolysis group and severe hemolysis group were significantly higher than those in non-hemolysis group,the above-mentioned indexes in severe hemolysis group were higher than those in mild to moderate hemolysis group,the difference was statistically significant(P<0.05).Conclusion The hemolysis sample used in the detection of liver function,electrolyte,blood lipid and myocardial enzyme indexes in biochemical tests will have a significant impact on the detection results.In order to ensure the accuracy of the detection results,it is necessary to use blood samples for detection.
