基于TTM的动机性访谈联合呼吸功能训练对肺癌患者肺功能锻炼依从性及生活质量的影响

Effect of motivational Interview based on TTM combined with respiratory Function Training on pulmonary Function Exercise Compliance and Quality of Life in Patients with Lung Cancer

目的探究基于跨理论模型(TTM)的动机性访谈联合呼吸功能训练对肺癌患者肺功能锻炼依从性及生活质量的的影响。方法选取河南省人民医院2020年1月至2021年10月收治的101例围术期肺癌患者作为研究对象,通过随机数字表法将其分为对照组和研究组,其中对照组50例行康复训练,研究组51例在对照组基础上联合基于TTM的动机性访谈进行干预。比较两组患者干预前后肺功能锻炼依从性、生活质量、焦虑/抑郁情绪、护理满意度及住院期间不良事件发生率。结果研究组肺功能锻炼依从性(96.08%)高于对照组(82.00%),差异有统计学意义(P<0.05)。两组干预后SF-36评分较干预前均升高,研究组高于对照组,差异有统计学意义(P<0.05)。两组干预后HAMA、HAMD评分较护理前均降低,研究组均低于对照组,差异有统计学意义(P<0.05)。研究组护理满意度(96.08%)高于对照组(84.00%),差异有统计学意义(P<0.05)。两组不良事件发生率比较差异无统计学意义(P>0.05)。结论基于TTM的动机性访谈辅助康复训练应用于肺癌患者中,更有助于提升患者肺功能训练依从性,减少负面情绪,提升生活质量。

Objective To explore the effect of motivational interview based on trans-theoretical model(TTM)combined with respiratory function training on the compliance of lung function exercise and quality of life of patients with lung cancer.Methods A total of 101 patients with perioperative lung cancer admitted to Henan people’s Hospital from January 2020 to October 2021 were selected as the study subjects.They were divided into control group and study group by random number table method.Among them,50 patients in the control group received rehabilitation training,and 51 patients in the study group received motivational interviews based on TTM on the basis of the control group.The compliance of pulmonary function exercise,quality of life,anxiety/depression,nursing satisfaction and incidence of adverse events during hospitalization were compared between the two groups before and after intervention.Results The compliance of pulmonary function exercise in the study group(96.08%)was higher than that in the control group(82.00%),and the difference was statistically significant(P<0.05).After the intervention,SF-36 scores of the two groups were higher than those before the intervention,the study group was higher than the control group,and the difference was statistically significant(P<0.05).The scores of HAMA and HAMD in the two groups after intervention were lower than those before nursing,and the scores in the study group were lower than those in the control group(P<0.05).The nursing satisfaction of the study group(96.08%)was higher than that of the control group(84.00%),and the difference was statistically significant(P<0.05).There was no significant difference in the incidence of adverse events between the two groups(P>0.05).Conclusion The application of motivational interview based on TTM assisted rehabilitation training in patients with lung cancer is more helpful to improve the compliance of patients with lung function training,reduce negative emotions and improve the quality of life.
