升清化浊方治疗肛窦炎湿热下注证疗效研究

Clinical study on curative effect of Shengqing Huazhuo prescription in treatment for anal sinusitis of Shire Xiazhu syndrome

目的:探讨升清化浊方治疗肛窦炎湿热下注证疗效。方法:选择90例肛窦炎湿热下注证患者,随机分为对照组、观察组,对照组予甲硝唑注射液、硫酸庆大霉素注射液混合溶液灌肠治疗,观察组在对照组基础上联合升清化浊方口服治疗,两组均连续治疗4周。比较两组治疗前后临床症状积分、血清炎症指标、临床疗效、复发情况、不良反应发生情况。结果:两组治疗后肛门坠胀、肛门瘙痒、肛门疼痛、肛窦充血、肛窦水肿症状积分以及TNF-α、IL-6、IL-2水平均降低,且观察组以上指标均低于对照组,差异有统计学意义(均P<0.05)。治疗后,两组血清IFN-γ水平升高,且观察组高于对照组,差异有统计学意义(均P<0.05)。观察组总有效率93.33%,高于对照组的75.56%(P<0.05)。观察组治疗后3、6个月的复发率均低于同时间段的对照组,差异有统计学意义(均P<0.05)。治疗期间两组均未发生明显不良反应。结论:升清化浊方治疗肛窦炎湿热下注证疗效较好,可抑制机体炎症反应,缓解临床症状,控制复发率,安全性较高。

Objective:To investigate curative effect of Shengqing Huazhuo prescription on anal sinusitis of Shire Xiazhu syndrome.Methods:90 patients with anal sinusitis of Shire Xiazhu syndrome were randomly divided into control group and observation group.The control group were treated with metronidazole injection gentamicin sulfate injection mixed solution enema,and the observation group were treated with Shengqing Huazhuo decoction.Both groups were treated for four weeks.The clinical symptom scores and serum inflammatory indexes were observed and compared between the two groups before and after treatment.Clinical efficacy,recurrence status and the occurrence of adverse reactions were compared between the two groups.Results:After treatment,the scores of symptoms of anal distension,anal pruritus,anal pain,anal sinus congestion,anal sinus edema,levels of TNF-α,IL-6 and IL-2 in the observation group were lower than those in the control group,difference statistically significant(all P<0.05).After treatment,serum IFN-γlevels in both groups were increased,and the observation group was higher than that in the control group,difference statistically significant(all P<0.05).The total effective rate in observation group was 93.33%,which was higher than 75.56%in the control group(P<0.05).After treatment in 3 and 6 months,recurrence rate of the observation group was significantly lower than that of the control group,difference statistically significant(P<0.05).There were no obvious adverse reactions in the two groups during treatment.Conclusion:Shengqing Huazhuo prescription has good curative effect in the treatment of anal sinusitis with Shire Xiazhu syndrome,which can inhibit inflammatory reaction of the body,relieve clinical symptoms,control recurrence rate,and has better safety.
