人性化护理联合体位护理对硬膜外分娩镇痛产妇出血量及预后的影响

Effect of humanized nursing combined with postural nursing on the bleeding volume and prognosis of puerperae undergoing epidural labor analgesia

目的 分析人性化护理联合体位护理在行硬膜外分娩镇痛产妇中的应用效果及对母婴预后的影响。方法 回顾性分析2019年10月到2021年6月盐城市妇幼保健院收治的82例行硬膜外分娩镇痛产妇的临床资料,按护理方法的不同分为对照组(42例)和观察组(40例)。对照组实施常规护理,观察组实施人性化护理联合体位护理。比较2组产妇的镇痛效果、产程时间、并发症发生率和出血量,以及围产儿情况。结果 观察组的疼痛缓解率(97.50%)高于对照组(83.33%,P<0.05)。与对照组相比,观察组的第一产程、第二产程、第三产程和总产程均更短(P均<0.05)。与同期对照组相比,观察组在产后2h及24h的出血量均更少(P均<0.05)。观察组产后出血、尿潴留及宫缩乏力的总发生率(5.00%)较对照组(19.04%)更低(P<0.05),胎儿宫内窘迫、新生儿窒息的总发生率(0.00%)也较对照组(9.52%)更低(P<0.05)。结论 对行硬膜外分娩镇痛的产妇采用人性化护理联合体位护理,对提高镇痛效果、缩短产程、减少产后出血及改善母婴结局均具有积极影响。

Objective To analyze the application effect of humanized nursing combined with postural nursing in puerperae undergoing epidural labor analgesia and its influence on the maternal and infant prognosis. Methods The clinical data of 82 puerperae undergoing epidural labor analgesia at Yancheng Maternity and Child Health Care Hospital from October 2019 to June 2021 were retrospectively analyzed. They were divided into a control group(42 cases)and an observation group(40 cases)according to different nursing methods. The control group received routine nursing,and the observation group received humanized nursing combined with postural nursing. The analgesic effect,labor duration,incidence of complications,bleeding volume of the puerperae,and the perinatal conditions were compared between the two groups. Results The pain relief rate of the observation group(97. 50%)was higher than that of the control group(83. 33%,P< 0. 05). The duration of the first,second,and third stage of labor,and the total duration of labor in the observation group were shorter than those in the control group(all P< 0. 05),and the bleeding volume at 2 and 24 hr after delivery was less than that in the control group(both P< 0. 05). The total incidence of postpartum hemorrhage,urinary retention,and uterine inertia in the observation group(5. 00%)was lower than that in the control group(19. 04%,P< 0. 05),and the total incidence of fetal distress and neonatal asphyxia( 0. 00%)was lower than that in the control group( 9. 52%,P< 0. 05).Conclusion Humanized nursing combined with postural nursing has a positive effect on improving the analgesic effect of puerperae undergoing epidural labor analgesia,shortening the labor duration,reducing the postpartum hemorrhage,and improving the maternal and infant outcomes.
