高强度间歇训练配合中医揿针对老年冠心病患者PCI术后生活质量的影响

摘要 目的探讨高强度间歇训练配合中医揿针对老年冠状动脉粥样硬化性心脏病(简称“冠心病”)患者接受经皮冠状动脉介入治疗(PCI)后心肺功能、运动能力及生活质量的影响。方法采用随机数字表法将2018年1月至2020年6月徐州医科大学附属医院收治的90例老年冠心病患者分为2组。对照组45例患者施以常规护理+中医揿针,观察组45例患者在常规护理+中医揿针的基础上施以高强度间歇训练。比较2组患者在入组时和干预24周后的心肺功能、运动能力和生活质量。结果干预24周后,2组的第1秒用力呼气容积(FEV_(1))、最大自主通气量(MVV)、无氧阈(AT)、最大摄氧量(VO_(2)_( max))、峰值氧脉搏(VO_(2)/HR)、二氧化碳通气当量(VE/VCO_(2))斜率均大于入组时(P均<0.05),且观察组的FEV_(1)、MVV、AT、VO_(2)_( max)、VO_(2)/HR、VE/VCO_(2)斜率均大于对照组(P均<0.05);2组的10 m最大步行速度测试(MWS)均慢于入组时,6分钟步行试验(6MWT)均长于入组时(P均<0.05),且观察组的MWS慢于对照组,6MWT长于对照组(P均<0.05);2组的心绞痛生活质量问卷(APQLQ)总分均高于入组时(P均<0.05),且观察组的APQLQ总分高于对照组(P<0.05)。结论对老年冠心病患者行PCI后实施高强度间歇训练联合中医揿针,可提高其心肺功能和运动能力,并显著提高其生活质量。 Objective To investigate the effect of high-intensity interval training combined with traditional Chinese medicine press needle on the cardiopulmonary function,exercise capacity,and quality of life of elderly patients with coronary atherosclerotic heart disease(referred to as coronary heart disease)after percutaneous coronary intervention(PCI).Methods Ninety elderly patients with coronary heart disease admitted to the Affiliated Hospital of Xuzhou Medical University from January 2018 to June 2020 were assigned into two groups by random number table method.The 45 patients in the control group were given routine nursing and traditional Chinese medicine press needle,and the 45 patients in the observation group were given high-intensity interval training on the basis of the routine nursing and traditional Chinese medicine press needle.The cardiopulmonary function,exercise capacity,and quality of life were compared between the two groups at baseline and after 24 weeks of intervention.Results After 24 weeks of intervention,the forced expiratory volume in 1 second(FEV_(1)),maximal voluntary ventilation(MVV),anaerobic threshold(AT),maximal oxygen uptake(VO_(2)_( max)),peak oxygen pulse(VO_(2)/HR),and minute ventilation/carbon dioxide production(VE/VCO_(2))slope in the two groups were greater than those at baseline(all P<0.05),and the FEV_(1),MVV,AT,VO_(2)_( max),VO_(2)/HR,and VE/VCO_(2) slope in the observation group were greater than those in the control group(all P<0.05).The 10-meter maximal walking speed(MWS)of the two groups was slower than that at baseline,and the 6-min walking test(6MWT)was longer than that at baseline(all P<0.05).The MWS of the observation group was slower than that of the control group,and the 6MWT was longer than that of the control group (both P <0. 05). The total scores of the Angina Pectoris Quality of Life Questionnaire (APQLQ) in the two groups were higher than those at baseline (both P <0. 05), and the total score of APQLQ in the observation group was higher than that in the control group (P < 0. 05). Conclusion High-intensity interval training combined with traditional Chinese medicine press needle can improve the cardiopulmonary function, exercise capacity, and quality of life of elderly patients with coronary heart disease after PCI.
