微弹簧圈联合明胶海绵在大咯血介入治疗中的应用效果

Application Effect of Micro-coil Combined with Gelatin Sponge in the Interventional Therapy of Massive Hemoptysis

目的:分析微弹簧圈联合明胶海绵在大咯血介入治疗中的应用效果。方法:选取2020年1月-2021年12月南京市溧水区人民医院收治的50例大咯血患者作为研究对象,按随机数字表法,分为对照组和观察组,各25例。两组均行明胶海绵治疗,观察组在对照组基础上增加微弹簧圈栓塞治疗。比较两组患者的止血情况、并发症、血管造影检查情况。结果:两组早期复发出血率比较,差异无统计学意义(P>0.05);观察组即刻止血率高于对照组、远期复发出血率低于对照组,差异有统计学意义(P<0.05)。两组并发症总发生率比较,差异无统计学意义(P>0.05)。两组栓塞治疗后的血管造影检查结果比较,差异无统计学意义(P>0.05)。结论:将微弹簧圈联合明胶海绵栓塞治疗应用到大咯血介入治疗中,能取得较理想的止血效果,且术后远期复发率低,可帮助患者维持良好的康复状态,值得推广应用。

Objective:To analyze the application effect of micro-coil combined with gelatin sponge in the interventional treatment of massive hemoptysis.Methods:From January 2020 to December 2021,50 patients with massive hemoptysis admitted to Nanjing Lishui District People's Hospital were selected as the research subjects.According to the random number table method,they were divided into control group and observation group,25 cases in each.Both groups were treated with gelatin sponge,and the observation group was additionally treated with micro-coil embolization on the basis of the control group.The hemostasis,complications and angiography of the two groups of patients were compared.Results:There was no significant difference in the early recurrence and bleeding rates between the two groups(P>0.05).The immediate hemostasis rate of the observation group was higher than that of the control group,and the long-term recurrence rate of bleeding was lower than that of the control group,and the differences were statistically significant(P<0.05).There was no significant difference in the total incidence of complications between the two groups(P>0.05).There was no significant difference in the angiography results between the two groups after embolization(P>0.05).Conclusion:The application of micro-coil combined with gelatin sponge embolization in the interventional treatment of massive hemoptysis can achieve ideal hemostasis effect,and the long-term recurrence rate after operation is low,which can help patients maintain a good recovery state,and is worthy of popularization and application.
