基于儿童早期预警评分的风险管理对NRDS患儿血清炎症因子表达及动脉血气水平的影响

摘要 目的:探讨基于儿童早期预警评分的风险管理对呼吸窘迫综合征新生患儿血清炎症因子表达及动脉血气水平的影响。方法:选取我科2018年10月至2020年10月期间91例新生儿呼吸窘迫综合征患儿,随机分为对照组和观察组,对照组45例给予常规护理干预,观察组46例给予儿童早期预警评分模式下的风险管理,于干预7d后,对比两组患儿的动脉血气指标、疾病风险指标、血清炎症因子及并发症发生情况。结果:干预7d后,观察组的动脉血氧分压、动脉血氧饱和度均高于对照组,动脉血二氧化碳分压低于对照组(P<0.05);观察组儿童早期预警评分、肺损伤评分均低于对照组(P<0.05);观察组的C-反应蛋白、白介素-10水平均低于对照组(P<0.05)。观察组并发症发生率低于对照组(P<0.05)。结论:基于儿童早期预警评分模式的风险管理有助于呼吸窘迫综合征新生患儿机体炎症的控制,改善患儿肺损伤和病情,进而促进动脉血气改善,减少并发症。 Objective:To investigate the effect of risk management based on the Childhood Early Warning Score(PEWS)on the expression of serum inflammatory factors and arterial blood gas levels in children with neonatal respiratory distress syndrome.Methods:A total of 91 children with neonatal respiratory distress syndrome in our department from October 2018 to October 2020 were selected and randomly divided into the control group and the observation group.45 cases in the control group were given routine nursing intervention,and 46 cases in the observation group were given PEWS in the risk management mode,after 7 days of intervention,the arterial blood gas indexes,disease risk indexes,serum inflammatory factors and complications were compared between the two groups.Results:After 7 days of intervention,the arterial blood oxygen partial pressure and arterial blood oxygen saturation in the observation group were higher than those in the control group,and the arterial blood carbon dioxide partial pressure was lower than the control group(P<0.05);the early warning score and lung injury score of the observation group were lower than those of the control group(P<0.05);the levels of C-reactive protein and interleukin-10 in the observation group were lower than those in the control group(P<0.05).The incidence of complications in the observation group was lower than that in the control group(P<0.05).Conclusion:Risk management based on the early warning scoring model of children can help control the inflammation of the body in children with neonatal respiratory distress syndrome,improve the lung injury and disease in children,and then promote the improvement of arterial blood gas and reduce complications.
