摘要
目的:研究基于营养支持小组(Nutrition support team,NST)管理模式对门脉高压行颈静脉肝内门体静脉分流术(Transjugular intrahepatic portal shunt,TIPS)患者的营养状况、肝性脑病发生情况的影响。方法:选取本院2020年9月至2021年10月期间105例行TIPS的门脉高压患者作为研究对象,随机分组,对照组52例给予常规饮食干预,观察组53例给予基于NST管理模式的营养支持,3 m后对比两组患者术后营养状况、人体成分、肝性脑病发生情况及再住院率。结果:观察组干预后总蛋白、血红蛋白、血清白蛋白水平均高于对照组(P<0.05);观察组干预后基础代谢率、身体脂肪比、蛋白总量、躯干水分和身体质量指数均高于对照组(P<0.05);观察组肝性脑病发生率和再住院率均低于对照组(P<0.05)。结论:基于NST管理模式的营养支持对行TIPS的门脉高压患者效果显著,能够有效改善患者营养状况,降低肝性脑病发生率和再住院率,具有临床推广价值。
Objective:To study the effect of the nutritional status and hepatic encephalopathy of patients undergoing intrahepatic portal body jugular vein shunt(TIPS)with portal hypertension based on nutritional support team(NST)management mode.Methods:A total of 105 TIPS patients with portal hypertension in our hospital from September 2020 to October 2021 were selected as the study subjects and divided into two groups by random number table method.52 patients in the control group received routine dietary intervention,and 53 patients in the observation group received nutritional support based on NST management mode.After 3 months,nutritional status,body composition,occurrence of hepatic encephalopathy and re-hospitalization rate of patients in the two groups were compared.Results:The levels of total protein,hemoglobin,and serum albumin in the observation group after intervention were higher than those in the control group(P<0.05);basal metabolic rate,body fat ratio,and total protein,trunk water and body mass index in the observation group after intervention were higher than those in the control group(P<0.05);the incidence of hepatic encephalopathy and readmission rate in the observation group were lower than those in the control group(P<0.05).
