老年腹股沟嵌顿疝患者急诊术前优质护理对患者预后影响的对比研究

Comparative study on the effect of high quality nursing before emergency operation on the prognosis of elderly patients with incarcerated inguinal hernia

目的探讨关于老年急诊患者出现腹股沟嵌顿疝危险因素评估,以及在急诊就诊过程中急诊优质护理措施对患者预后的影响。方法回顾性研究分析2017年1月至2019年12月,首都医科大学附属北京朝阳医院急诊科就诊的老年嵌顿疝患者72例。按照护理方法不同,将患者分为2组,每组患者36例。对照组患者采用急诊常规疝手术前护理流程,观察组给予急诊老年嵌顿疝术前优质护理。比较2组患者手术指标及术后指标。结果观察组患者的术后下床时间及术后并发症(血肿、血清肿、急性疼痛、感染)发生率明显低于对照组,差异有统计学意义(P<0.05)。对照组患者术后总有效率为87.9%,观察组为98.1%,差异有统计学意义(P<0.05)。对照组护理满意率为86.11%,观察组为97.22%,差异有统计学意义(P<0.05)。结论老年腹股沟嵌顿疝患者在急诊实施优质护理措施对于提升患者满意度等有显著的效果,对于老年腹股沟嵌顿疝患者的恢复有明显促进作用。

Objective To explore the risk factors of incarcerated inguinal hernia in elderly emergency patients,and the impact of high quality emergency nursing measures on the prognosis of patients.Methods A retrospective study was conducted to analyze 72 elderly patients with incarcerated hernia who were admitted to the emergency department of Beijing Chaoyang Hospital Affiliated to Capital Medical University from January 2017 to December 2019.They were divided into two groups according to nursing methods,with 36 patients in each group.The control group was given emergency routine nursing before operation,and the observation group was given emergency high quality nursing before operation.The operation indexes and postoperative indexes were compared between the two groups.Results The time of getting out of bed and the incidence of postoperative complications(hematoma,seroma,acute pain,infection)in the observation group were significantly lower than those in the control group,and the differences were statistically significant(P<0.05).The total effective rate in the control group was 87.9%,while the total effective rate in the observation group was 98.1%,and the difference was statistically significant(P<0.05).The nursing satisfaction rate of the control group was 86.11%,and that of the observation group was 97.22%,the difference was statistically significant(P<0.05).Conclusion The implementation of high quality nursing in the emergency department for elderly patients with incarcerated inguinal hernia has a significant effect on improving patients’satisfaction,and can promote the recovery of elderly patients with incarcerated inguinal hernia.
