基于行为改变理论的健康教育模式在乳腺癌内分泌治疗患者护理中的应用效果 被引量:1

摘要 目的 探讨基于行为改变理论的健康教育模式在乳腺癌内分泌治疗患者护理中的应用效果,以期为相关治疗与护理提供参考。方法 选取2019年2月-2020年8月于平顶山市第一人民医院完成手术、术后化疗并行内分泌用药治疗的女性乳腺癌病例118例为研究对象。采用随机数字表法将患者分为对照组与研究组,每组59例。对照组患者按照乳腺癌内分泌治疗的常规护理进行干预。研究组患者在对照组基础上采用基于行为改变理论的健康教育模式干预。采用自制的内分泌治疗用药知识、技能掌握情况量表评估2组患者干预前及出院时用药知识、用药技能情况。采用Morisky药物依从性问卷评估2组患者干预6个月时的用药依从性。出院时采用自制的护理满意度调查问卷调查2组患者护理满意度。结果干预前,2组患者用药知识、技能评分比较,差异无统计学意义(P>0.05)。出院时,2组患者用药知识、技能评分均高于干预前,且研究组高于对照组,差异均有统计学意义(P<0.05)。研究组患者用药总依从率为91.53%,高于对照组的79.66%,差异有统计学意义(P<0.05)。研究组患者护理满意度调查问卷中护患沟通、用药宣教、用药问题解决的及时性、用药服务有效性评分均高于对照组,差异均有统计学意义(P<0.05)。结论 对乳腺癌内分泌治疗患者进行基于行为改变理论的健康教育模式的护理干预,可提高患者相关知识、技能的掌握度,提高患者的用药依从性及护理满意度,值得临床推广应用。 Objective To explore the effect of the behavioral change-based health education model in nursing of patients with breast cancer receiving endocrine therapy,so as to provide guidance for related treatment and nursing.Methods A total of 118 female breast cancer patients who underwent surgery,postoperative chemotherapy,and endocrine therapy in the First People’s Hospital of Pingdingshan from Feb.2019 to Aug.2020 were selected as the study subjects.The patients were assigned to the control group and experimental group by a random number table,with 59 cases in each group.Patients in the control group received routine nursing for endocrine therapy against breast cancer.The patients in the experimental group received behavior change-based health education model intervention on top of the intervention in the control group.The Selfmade Endocrine Therapy Medication Knowledge Scale and Skill Mastery Scale were used to assess the medication knowledge and medication skills of patients in the two groups before intervention and at discharge.The Morisky Medication Adherence Scale was used to assess medication adherence at 6 months of intervention in the 2 groups.A self-made nursing satisfaction survey questionnaire was used at discharge to investigate the nursing satisfaction of patients in the 2 groups.Results Before the intervention,there was no significant difference in medication knowledge and skill scores between the two groups(P>0.05).At discharge,the medication knowledge and skill scores of the patients in the 2 groups were higher than those before the intervention,and the experimental group was higher than the control group,and the differences were statistically significant(P<0.05).The overall compliance rate of the experimental group was 91.53%,which was higher than that of the control group(79.66%),and the difference was statistically significant(P<0.05).The scores of nurse-patient communication,medication education,timeliness of medication problem solving,and effectiveness of medication services in the nursing satisfaction questionnaire in the experimental group were higher than those in the control group,and the differences were statistically significant(P<0.05).Conclusion Nursing intervention of behavior change-based health education model in patients with breast cancer who underwent endocrine therapy can improve the mastery of relevant knowledge and skills of patients,and raise medication compliance and nursing satisfaction.These benefits hold promise for a wider application of this procedure clinically.
