胰腺导管内乳头状黏液性肿瘤超声检查与MRI诊断效能比较研究

Comparative study on diagnostic efficiency of ultrasound and MRI for intraductal papillary mucinous neoplasm of pancreas

目的探讨建立一种新型评分方法,比较超声检查与MRI对胰腺导管内乳头状黏液性肿瘤(IPMN)的诊断效能。方法选取我院诊治的37例胰腺导管内乳头状黏液性肿瘤,根据评分标准分别对术前超声检查(32例)与MRI检查(28例)诊断报告的定位诊断、定性诊断赋予一定分值,并采用非参数秩和检验进行统计分析。结果术前超声检查定位诊断正确率为78.13%(25/32),MRI定位诊断正确率为91.67%(22/28),MRI定位诊断的平均秩高于超声检查。超声检查与MRI定性诊断的平均秩分别为26.70与30.90,综合诊断的平均秩分别为26.75与30.82,差异均无统计学意义(P>0.05)。结论评分法可较客观、全面、准确地评价胰腺IPMN的影像诊断效能,MRI检查诊断效能高于超声检查。

Objective To establish a scoring method for comprehensively comparing the diagnostic efficacy of ultrasound and MRI for intraductal papillary mucinous neoplasm(IPMM)of pancreas.Methods Among the included 37 cases of(IPMN)diagnosed and treated in our hospital,32 cases received preoperative ultrasound examination and 28 received MRI examination.The ultrasound and MRI diagnostic reports were assigned scores according to the scoring method for localization and qualitative diagnosis,respectively,and further analyzed by non-parametric rank sum test.Results The accuracy of preoperative ultrasound and MRI localization diagnosis was 78.13%(25/32),91.67%(22/28),respectively;the average rank of MRI was higher than ultrasound.The average ranks of qualitative diagnosis of ultrasound and MRI were 26.70 and 30.90,respectively;the average ranks of comprehensive diagnosis were 26.75 and 30.82,respectively,and there was no statistically significant difference(P>0.05).Conclusion The scoring method can objectively,comprehensively and accurately evaluate the imaging diagnostic effectiveness of pancreatic IPMN and the diagnostic effectiveness of MRI is higher than that of ultrasound.
