超声全胃圆柱体法对终末期肾功能不全患者胃排空的评估分析

Evaluation and analysis of ultrasonic whole stomach cylinder method on gastric emptying in patients with end-stage renal dysfunction

目的探讨经超声全胃圆柱体法(UWSCM)评估终末期肾功能不全患者胃排空时间的价值,为未来该类疾病临床诊疗方案的制定提供参考依据。方法选取我院接受治疗的30例终末期肾功能不全患者,使用上消化道症状评分评估全部入选者的上消化道症状,分别使用UWSCM、放射性核素显像法、超声胃窦面积法、超声胃窦体积法检查并计算全部入选者胃排空25%(T1)、胃排空50%(T2)、胃排空75%(T3)所用时间,分析UWSCM检测各阶段胃排空时间与上消化道症状评分间的相关性;并经组内相关系数(ICC)分析UWSCM检测各阶段胃排空时间与放射性核素显像法的一致性。结果UWSCM与放射性核素显像法检查各阶段胃排空时间相近(P>0.05);超声胃窦面积法、超声胃窦体积法检查各阶段胃排空时间均分别短于UWSCM与放射性核素显像法(P<0.05);采用双变量Pearson直线相关性分析,结果显示UWSCM检测各阶段胃排空时间与上消化道症状评分之间呈正相关(r>0,P<0.05);采用ICC分析,结果显示UWSCM检测各阶段胃排空时间与放射性核素显像法检测各阶段胃排空时间一致性较好,ICC值分别为0.970、0.989、0.949(P<0.001)。结论采用UWSCM评估终末期肾功能不全患者各阶段胃排空时间与“金标准”放射性核素显像法检查所用时间相近,具有较高的应用价值。

Objective To explore the value of time of ultrasonic whole stomach cylinder method(UWSCM)on the evaluation of gastric emptying in patients with end-stage renal dysfunction,so as to provide reference for the formulation of clinical diagnosis and treatment plan for this kind of disease in the future.Methods 30 patients with end-stage renal dysfunction treated in the hospital were selected.The upper gastrointestinal symptom score was used to assess the upper gastrointestinal symptoms.Their time of gastric emptying 25%(T1),gastric emptying 50%(T2),or gastric emptying 75%(T3)was calculated by UWSCM,radionuclide imaging,ultrasonic gastric antrum area method,or ultrasonic gastric antrum volume method.The correlation between gastric emptying time at different stages by UWSCM examination and upper gastrointestinal symptom score was analyzed;the consistency of gastric emptying time at different stages determined by UWSCM and the radionuclide imaging method were analyzed by intra-group correlation coefficient(ICC).Results The time of UWSCM and radionuclide imaging was similar in the examination of gastric emptying time at different stages(P>0.05);The detection on gastric emptying time at different stages by ultrasonic gastric antrum area method,ultrasonic gastric antrum volume method was shorter than that of UWSCM and radionuclide imaging(P<0.05);bivariate Pearson linear correlation analysis was used,and the results showed a positive correlation between gastric emptying time at different stages by the UWSCM test and upper gastrointestinal symptom scores(r>0,P<0.05);ICC analysis was used,the results showed that the consistency between the gastric emptying time at different stages by UWSCM examination and gastric emptying time at different stages by radionuclide imaging examination were good,the ICC values was 0.970,0.989,and 0.949,respectively(P<0.001),with good repeatablity.Conclusion The gastric emptying time at different stages of patients with end-stage renal insufficiency assessed by UWSCW is similar to that of“gold standard”radionuclide imaging method,which has high application value.
