小肠克罗恩病CTE表现及赋能教育的应用效果

CTE manifestation and effect of enabling education in patients with small intestinal Crohn's disease

目的探讨多层螺旋CT小肠成像(multi-slice computed tomography enteroclysis,CTE)在小肠克罗恩病(Crohn's disease,CD)诊断中的表现及赋能教育的临床应用价值。方法选取我院44例小肠CD患者,回顾性分析其临床资料,通过赋能教育,分析患者CTE的成像质量、检查成功率,比较干预前后患者自我效能感和生活质量的差异。结果44例患者均顺利完成CTE检查,其中41例患者成像质量效果优,且干预前后患者的自我效能、生活质量差异具有统计学意义(P<0.05)。克罗恩病CTE表现为回肠远端肠壁增厚呈多发节段性40例(90%),增强扫描后呈分层强化32例(73%);系膜侧血管分支增多,呈梳征20例(45%)。结论CET能较好地显示CD患者肠道特征,在小肠CD患者中采取赋能教育可提升CTE成像质量,显示肠内肠外疾病特征,提高诊断成功率,增强其自我效能,改善患者生活质量。

Objective To investigate the clinical application of multi-slice computed tomography enteroclysis(CTE)in the diagnosis of Crohn's disease(CD)and the value of empowerment education.Methods 44 patients with small intestinal CD in our hospital were selected and their clinical data were retrospectively analyzed.The imaging quality and success rate of CTE were analyzed through empowerment education,and the differences of self-efficacy and quality of life of patients before and after intervention were compared.Results All 44 patients successfully completed CTE examination,among whom 41 patients had excellent imaging quality effect,and the differences of self-efficacy and quality of life before and after intervention were statistically significant(P<0.05).CTE in Crohn's disease was characterized by multiple segmental thickening of the distal ileum wall in 40 cases(90%),and stratification enhancement after enhanced scanning in 32 cases(73%).There were more vascular branches in mesangial side and 20 cases(45%)showed comb sign.Conclusion Enabling education in patients with small intestinal CD can improve the imaging quality of CTE,display the characteristics of enteral and parenteral diseases,improve the diagnostic success rate,enhance self-efficacy,and improve the quality of life of patients.
