兔急性痛风性关节炎模型320排容积CT的灌注成像研究

Study of 320-slice dynamic volume CT perfusion on acute gout arthritis of knee-joint in rabbit

目的探讨分析兔急性痛风性关节炎模型容积CT灌注成像表现。方法制作急性痛风性关节炎动物模型。选取新西兰大白兔35只,随机将实验动物分为实验组(n=30只,右膝关节注射尿酸钠溶液及多黏菌素),对照组(n=5只,注射生理盐水及多黏菌素);实验组按给药后4、16、32、48、60、72 h时间段分为6组,每组5只;实验组于不同时间给药后行CT灌注扫描,绘制时间-密度(TDC)曲线,分析其血流量(BF)、血容量(BV)、血管通透性(PS)、膝关节滑膜微血管密度(MVD)。灌注成像完成后处死实验动物,取其膝关节囊软组织做病理切片。结果实验组BF、BV、PS及MVD明显高于对照组,差异有统计学意义(P均<0.05);实验组BF、BV、PS及MVD在不同时间点均差异有统计学意义(P均<0.05),实验组给药4、16、32 h,BF、BV、MVD呈缓慢上升趋势,在32 h达峰值;在32、48 h呈较平稳高峰趋势,48~72 h呈缓慢下降趋势;PS在给药4~48 h间呈上升趋势,在48 h达峰值;在48~72 h呈缓慢下降趋势。结论CT灌注成像能够较全面反映兔急性痛风性关节炎模型的血流动力学变化。

Objective To analyze volume CT perfusion imaging of acute gout arthritis of knee-joint in rabbits.Methods The acute gout arthritis rabbit model was prepared,and then,35 rabbits were randomly divided into two groups,including experimental group(n=30)and control group(n=5,injected with saline and polymyxin).The experimental group was injected with sodium urate solution and polymyxin,then divided into six groups respectively after 4,16,32,48,60,72 h.Rabbits in the experimental group and control group all underwent CT perfusion scan.Time-density curve(TDC)was depicted and the values of blood flow(BF),blood volume(BV),permeability surface(PS),and micro vessel density(MVD)were analyzed.Rabbits were sacrificed after perfusion imaging,and the knee-joint was taken for pathologic examination.Results The BF,BV,PS,or MVD in experimental group was significantly higher than that in control group(all P<0.05),BF,BV,PS and MVD were significantly different among different time points(all P<0.05).BF,BV,MVD increased slowly 4,16,32 h after administration,increased to peak at 32 h.At 32,48 h,the BF,BV and MVD were in relatively stable trend,decreased slowly at 48~72 h.and then,PS showed a slow a scent at 4~48 h,and increased to peak at 48 h,and decreased slowly at 48~72 h.Conclusion CT perfusion imaging can reflect the blood changes of acute gout arthritis of knee-joint in rabbit wholely.
